Objective:To investigate the effect of medical nutrition intervention in preventing or delaying the progress of gestational diabetes mellitus,we detected the change of interleukin-6(IL-6),fasting insulin(FI),C reactive protein(CRP),adiponectin,(ADPN),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),cholesterol(TC),fasting plasma glucose(FPG)and other indicators before and after medical nutrition therapy.This is to explore medical nutrition therapy to reduce the incidence of gestational diabetes mellitus,to further clarify the correlation between medical nutrition therapy and the development of gestational diabetes mellitus,and clarified on the mechanism of gestational diabetes mellitus,and provided reference for the application of nutrition intervention treatment for clinical medicine of gestational diabetes mellitus.Method:We selected 402 cases randomly from the obstetric clinic for 24-28w weeks of gestation of pregnant women with gestational diabetes.Among them,210 cases with agreed with medical nutrition intervention as experimental group;192 cases do not agree with medical nutrition therapy as the control group.The experimental group was given medical nutrition therapy on the basis of routine nursing.The control group was given routine nursing care.Patients with gestational diabetes compare IL-6,FI,CRP,ADPN,TQ HDL-C,LDL-C,TC,FPG and other related changes of test indexes before and after medical nutrition intervention,in order to discuss the medical nutrition intervention in preventing or delaying the occurrence and development of gestational diabetes.Results:1.Indicators between experimental group before intervention and control group Compared with control group,the difference of fasting insulin,high-density lipoprotein cholesterol and fasting blood glucose in the experimental group before intervention was statistically significant(P<0.05).The levels of fasting insulin and fasting blood glucose in the experimental group were significantly higher than the control group(P<0.05).The level of high density lipoprotein cholesterol in the experimental group was significantly lower than the control group(P<0.05).The levels of IL-6,C reactive protein,adiponectin,triglyceride,low density lipoprotein cholesterol,and total cholesterol was no obvious difference between the two groups(P>0.05).2.The change of indicators in experimental group before and after intervention After the intervention,the levels of fasting blood glucose and fasting insulin in experimental group had reductions.The reduction of fasting blood glucose was statistically significant(P<0.05),and the fasting insulin levels had borderline significance(P<0.1).The level of interleukin-6,c-reactive protein,adiponectin,triglyceride,low density lipoprotein cholesterol,total cholesterol,and high density lipoprotein cholesterol had no significant change(P>0.05).3.The change of indicators between the control group and experimental group after interventionCompared with control group,the levels of fasting insulin,high density lipoprotein cholesterol,and fasting glucose from experimental group had after intervention.Among them,the reduction of fasting insulin and fasting glucose was statistically significant(P<0.05).Interleukin-6,c-reactive protein,adiponectin,triglyceride,low density lipoprotein cholesterol,total cholesterol,and high density lipoprotein cholesterol have no significant change(P>0.05).Conclusion:l.The levels of fasting insulin and fasting blood glucose in experimental group was significantly higher than the control group,while the level of high density lipoprotein cholesterol was significantly lower than the control group.We found the fasting blood glucose and fasting insulin were associated with gestational diabetes mellitus,and high density lipoprotein cholesterol was a protective factor for gestational diabetes.Patients with gestational diabetes mellitus may be more sensitive to the level of high density lipoprotein cholesterol.It indicates that fasting insulin,high density lipoprotein cholesterol and fasting blood glucose are closely related to the occurrence and development of gestational diabetes mellitus.2.There is no significant difference in the levels of interleukin-6,C reactive protein,adiponectin,triglyceride,low density lipoprotein cholesterol,and total cholesterol between experimental group and control group.It suggests that the levels of interleukin-6,C reactive protein,low density lipoprotein cholesterol and total cholesterol were not correlated to the occurrence and development of gestationaldiabetes mellitus.3.After intervention,levels of fasting blood glucose and fasting insulin in the experimental group significantly decreased compared with that before the treatment.There was significant difference in the fasting blood glucose level,and borderline significance in the fasting insulin level.It shows that medical nutrition intervention in gestational diabetes can effectively control the blood glucose levels,improve glucose and lipid metabolism,and effectively prevent or delay the occurrence and development of gestational diabetes mellitus.
