Clinical Observation Of Small Needle Knife In The Treatment Of Vertibular Peripheral Vertigo Acute Attack

Purpose:To observe the clinical effect of small needle knife in the treatment of vertibular perphral vertigo acute attack,to understand the effective rate of small needle knife on the acupoints of the posterior part to treat vertibular peripheral vertigo acute attack,and to explore the optimal indication for vertibular peripheral vertigo acute attack with small needle knife.Methods:192 patients with acute attack of vestibular peripheral vertigo in accordance with the inclusion criteria were enrolled.The subjects were randomly divided into the experimental group and the control group by SAS statistical software.The patients treated with the small needle knife were the experimental group,and the patients treated with diphenhydramine were the control group.The scores of the vestibular symptom index(VSI),change in the Dizziness Scale(DHI)scores and the scoring system for the degree of vertigo assessment(DARS)were observed before and after treatment in the experimental group and the control group at 5 min,30 min,2 h,6 h,24 h,72 h,and 6 d after treatment.Statistical analysis was performed based on the last statistical data.Results:1.The subjects were 192 patients who were treated with “dizziness”as the main complaints of Chengdu University of Traditional Chinese Medicine,Chongqing Traditional Chinese Medicine Hospital,and Jinniu District Hospital of Traditional Chinese Medicine.the experimental group exclude 1 case,and 3 patients in the control group were excluded.A total of 188 cases were analyzed finally,including 63 males and 125 females;71 Meniere's disease,15 migraine vertigo,7 hunt's syndrome,19 vestibular neuronitis,and 55 sudden deafness with vertigo,21 cases of other vestibular peripheral vertigo;2.Among the subjects included in the study,65(35%)were deficiency of both qi and blood syndrome,31 cases(16.5%)were deficiency and exhaustion of Liver-Yin syndrome,and 30 cases were phlegm stagnates collateral syndrome.(16%),29 cases(15.5%)were on the wind-Phlegm stagnates collateral,12 cases(11.5%)had kidney-Essence deficiency syndrome,and 11 cases(5.5%)had fire caused by hyperactivity of Liver-Yang.3.Comparison the vestibular symptom index(VSI)scores of the experimental group before treatment-5minutes after treatment,there was a statistically significant difference(P < 0.01).The efficacy tends to be stable at 30 min,2hmin,6h,24 h,72h,6d.The total effective rate of symptom improvement in the experimental group 5 minutes after treatment was 66.3%;4.Comparison of the efficacy of small needle knife treatment and oral diphenhydramine treatment,the experimental group was better than that of the control group.5.In the experimental group,the independent scores of vestibular symptoms were significantly lower than those before treatment at 5 min after small needle knife treatment,P<0.01,and the difference was statistically significant.6.Comparison the DARS scores of the experimental group before treatment-5minutes after treatment,there was a statistically significant difference(P < 0.01).7.Comparison the DHI scores of the experimental group before treatment-5minutes after treatment,there was a statistically significant difference(P < 0.05).8.The scores of emotion(E),function(F)and physical(P)in the experimental group were significantly lower than those before treatment at 5 min after the small needle knife treatment,and P < 0.01.The difference was statistically significant.Conclusions:1.The improvement of vestibular symptoms and the reduction of vertigo in the acute attack of vestibular peripheral vertigo after treatment with small needle knife is better than that of diphenhydramine group;2.The time of onset of small needle knife treatment is basically within 5 minutes.With the prolongation of observation time window,the patient's symptoms are continuously improved or remain stable.If the symptoms of the patient are not improved significantly within 5 minutes,the patient's symptoms of vertigo will be no further relieved with time.The results show that the effect of the small needle knife is fast and stable;3.Small needle knife treatment of acute attack of vestibular peripheral vertigo can significantly improve the patient's emotion,function and quality of life;4.The treatment of acute attack of vestibular peripheral vertigo by small needle knife can quickly and significantly improve the subject's balance disorder,dizziness,nausea and other related symptoms.
