| Objective:To investigate the effectiveness of lidocaine injection behind the ear in the treatment of acute attacks of vestibular migraine(VM)vertigo,the improvement of the emotional,functional,and physical damage of VM patients,and its beneficial effects on improving the quality of life of patients.Methods: According to the diagnostic criteria in the The International Classification of Headache Disorders,third edition(ICHD-Ⅲ)in 2018 and the "Expert Consensus on the Diagnosis and Treatment of Vestibular Migraine(2018)" published in China,the period from December 2019 to December 2020,patients with vertigo acute attack of VM in the outpatient department of Otolaryngology Head and neck surgery center of Dalian Central Hospital Affiliated to Dalian Medical University or in the hospital department were selected and randomly divided into group A and group B,each group enrolled 25 patients,and the lifestyle of all patients was integrated management,group A was the observation group and was given flunarizine hydrochloride capsules(sibelin)10mg,orally before going to bed every day,betahistine mesylate tablets(minshilang)12mg,tid,retroauricular injection of 2% lidocaine 5ml,qd,group B is the control group given sibelin 10 mg,orally before going to bed daily,minshilang 12 mg,tid,two groups of patients treat with medication until the dizziness is completely relieved or continue medication for 72 hours.The visual analogue scale(VAS)was used to evaluate the changes in the severity of vertigo before treatment,5min,30 min,24h,48 h,72h after treatment.The Dizziness Rating Scale(DARS)was performed before treatment and 72 hours after treatment to assess the changes in the patient’s condition.The effects of dizziness on mood,function and physical damage(Dizziness Disorder Inventory,DHI)were observed at two points before treatment and 72 hours after treatment.Observe and record the occurrence of adverse events throughout the treatment process,and perform statistical analysis based on the data results.Results:A total of 45 patients in the 50 patients completed the follow-up,24 in group A and 21 in group B.There was no significant difference in VAS score,DARS score and DHI score between the two groups before treatment(P>0.05),and they were comparable;Both groups A and B can alleviate the severity of dizziness in patients,and the trend of changes in each time window is statistically different(P<0.01);on the basis of oral drug treatment,patients in group A received lidocaine injection behind the ear Symptoms can begin to relieve after 5 minutes,and the effect is obvious at 30 minutes,while oral drug treatment in group B is more effective after 24 hours;compared with the treatment plan of group B,the treatment plan of group A is more effective and quick to relieve the symptoms of dizziness in patients.There was a statistically significant difference in the degree of dizziness relief between the two groups(P<0.01);the scoring system(DARS)score of the dizziness assessment degree of the patients in the two groups was significantly changed 72 h after the treatment(P<0.01);the treatment of the two groups The impact of the protocol on the scoring system(DARS)of the patient’s vertigo rating scale was statistically significant(P<0.01);patients in groups A and B were assessed 72 hours after the end of treatment to assess the impact of the acute attack of vertigo on the degree of emotional,function and physical damage(DHI scores)were significantly improved(P<0.05),and there was no significant difference in the treatment plan between the two groups(P>0.05).Conclusion:1.Patients with acute vertigo attack of vestibular migraine can significantly alleviate the symptoms of vertigo after treatment with retroauricular injection of lidocaine.2.In the case of comprehensive management of patients’ daily life style,patients with acute vertigo attack of vestibular migraine have obvious improvement in mood,function and quality of life after treatment with retroauricular injection of lidocaine. |
