Objective:The purpose of this study was to investigate the three-dimensional changes of alveolar bone after immediate implant of traumatic tooth loss in aesthetic area,and to analyze the influencing factors and the changing regularity of these changes,so as to provide reference and clinical guidance for immediate implant in aesthetic area post-traumatic.Methods:According to the inclusion criteria,a total of 59 implants,including 20 males and23 females who were admitted to the first affiliate hospital of NanChang University in aesthetic area post-traumatic were selected from 2016.6 to 2017.12.In group A,28 implants with intact labial bone plate were implanted with bone graft into jumping gap,and in group B,31 implants were used graft and membrane to restore labial bone plate defect.All missing teeth were restored finally 6 months later.CBCT imaging data of the patients on the day of implantation completion(0 months),restoration completion(6 months),and 6 month after restoration(12 months)respectively were obtained.The height of alveolar bone at 4 sites,including labial(LH),palatal(PH),mesial(MH)and distal(DH),and the thickness of labial palate at the site of 0mm(LT0),2mm(LT2),4mm(LT4),6mm(LT6),8mm(LT8)from the implant shoulder to apical were measured.The changes of alveolar bone of peri-implants in the two groups at different time points were analyzed by repeated measurement variance method.Results:During the the observation period,the success rate of implant and restoration were 100%.The thickness of labial alveolar bone at the site of LT0?LT2?LT4?LT6?LT8 were statistically significant according to repeat measurement and varianceanalysis between groups and at different time(P<0.05).But there was no significant difference at the site of LT8 between groups and there was no interaction at the same time between groups.After the healing period of 6 months,the thickness of labial alveolar bone of both groups at the site of LT0?LT2?LT4?LT6?LT8 were absorbed significantly.The resorption of group B at the site of LT0?LT2?LT4?LT6 which were2.02±0.57 mm,2.12±0.46 mm,0.99±0.35 mm,0.93±0.17 mm were higher than the resorption of group A which were 1.78±0.47 mm,1.73±0.73 mm,0.68±0.47 mm,0.68±0.27 mm with significant difference from T0 to T1(P < 0.05).There were no significant difference at all sites between T0 toT1 and T0 to T2 both groups(P>0.05).According to repeat measurement and variance analysis,There was significant difference at the site of LH(P<0.05),but there were no significant difference at the site of PH,MH,DH between groups(P>0.05).There was significant difference at the site of LH(P<0.05),but there were no significant difference at the site of PH,MH,DH between groups at different time(P>0.05).there was no significant difference between groups and there was no interaction at the same time between groups.After the healing period of 6 months,the height of labial alveolar bone of both groups at the site of LH were absorbed significantly.The resorption of group B at the site of LH which was 1.10±0.29 mm was higher than the resorption of group A which was 0.70±0.14 mm with significant difference from T0 to T1(P<0.05).There were no significant difference at the site of PH,MH,DH between T0 toT1 and T0 to T2 both groups(P>0.05)?Conclusion:1 Immediate implant in aesthetic area post-traumatic can achieve success,so it is worthy promoting for clinic.2 After immediate implant in aesthetic area post-traumatic,the labial palate(height and thickness)will be absorbed,and aesthetic risk exists.3 Intact labial palate can reduce alveolar bone resorption after immediate implant,and integrity of labial alveolar bone should be preserved as far as possible during operation.
