| Objective:To compare the effect of placing the definitive restorative abutment or the temporary abutment on the soft and hard tissue during implant surgery in a single upper anterior tooth.Methods:Thirty-six patients with single upper anterior tooth loss who visited the Department of Prosthodontics of our hospital were randomly divided into the test group and the control group.In the test group,a definitive restorative abutment was placed after implant placement;in the control group,a temporary abutment was placed after implant placement,the Ankylos implant system was used in both groups,and immediate restoration was performed after implant placement in both groups.Six months after implant placement,the test group was restored with all-ceramic crowns at the abutment level;the control group was restored with all-ceramic crowns at the implant level and replaced with restorative abutments.In the test group and the control group,marginal bone level changes,the rink esthetic score(PES),probing depth(PD)and bleed on probing(BOP)index were measured on the day of all-ceramic crown insertion and 6 months after all-ceramic crown insertion,respectively,and the satisfaction of the patients was investigated and compared 6 months after all-ceramic crown restoration.Results:There was no significant difference in marginal bone level changes between the test group and the control group on the day of all-ceramic crown insertion and 6 months after all-ceramic crown insertion(P>0.05).The PES score was compared between the test group and the control group on the day of all-ceramic crown insertion and 6 months after all-ceramic crown insertion,and there was a significant difference between the two groups(P<0.05).The patient satisfaction was investigated in the test group and the control group at 6 months after all-ceramic crown insertion,and the satisfaction of the test group was significantly different between the two groups(P<0.05).The bleeding index of visiting depth exploration was measured on the day of all-ceramic crown insertion and 6 months after all-ceramic crown insertion in the test group and the control group,and there were no significant differences between the two groups(P>0.05).Conclusion:Placement of a definitive restorative abutment at the time of implant surgery in the esthetic zone of the upper anterior teeth is an effective immediate restorative method.In early anterior implants,immediate restoration with a definitive restorative abutment at the time of implant surgery showed no difference in marginal bone level changes compared with conventional immediate restoration methods,and immediate restoration with a definitive restorative abutment resulted in higher pink esthetic scores and patient satisfaction.However,the long-term marginal bone resorption and esthetic results of implants still need to be observed in sample size and longer follow-up. |
