ObjectiveOn the basis of previous studies, this paper takes filiform needle shallow needling as an intervention measure and adopts the real world research method to explore the influence of frequency of intervention measures on the curative effect of cervical spondylosis cervical pain,to explore the best frequency of acupuncture, and to improve the scheme of acupuncture treatment of cervical spondylosis cervical pain.MethodsAll subjects were recruited from the outpatient department of acupuncture and moxibustion of Guangdong Hospital of Traditional Chinese Medicine from May 2018 to December 2018.A total of 105 cases were included.According to the principle of real world research, patients were divided into three groups according to their actual condition and willingness. They were divided into five groups per week, three groups per week and two groups per week for intervention. Finally a total of 101 effective cases were included. The treatment evaluation period was 2 weeks and the follow-up period was 3 months after the treatment.NPQ Neck Pain Scale and Simple McGill Pain Scale were collected before treatment,1 week after treatment,2 weeks after treatment,1 month after follow-up and 3 months after follow-up.The collected data were analyzed by SPSS18.0 software, and the test level was a=0.05.The measurement data were expressed as normal distribution and homogeneity of variance. The Wilcoxon rank sum test was used for the comparison between groups, and the repeated measurement analysis was used for the comparison of the data measured at each observation point. The covariance matrix and sphericity test were not met.The G-G model was used.Classified data are expressed by composition ratio and rate.Chi-square test or Fisher' s exact probability method are used.Wilcoxon rank sum test(correction)for sample comparison or Kruskal-Wallis test for multiple comparisons were used for ranking data.Results(1)Baseline comparisonChi-square test or variance analysis was performed on age, sex, occupation, education level, time to keep head down, course of disease, habits of sleeping position with high pillow,TCM syndrome differentiation and scale scores of the three groups. The results showed no significant difference(P>0.05),suggesting that the baselines of the three groups were equal and comparable.(2)Main Outcome Indicators:Northwick Park Neck Pain Scale(NPQ)score(a)Intra-group comparison:After treatment,NPQ scores of the three groups decreased significantly at each observation point(P<0.05),suggesting that different frequency of superficial needling with filiform needles has clinical efficacy in improving other functional disorders caused by cervical spondylosis.(b)Inter-group comparison:After treatment, the NPQ scores of the three groups were reduced. The improvement of NPQ scores of the five groups and three groups were better than that of the two groups(P<0.001 and P=0.017<0.05).The difference of NPQ scores between the five groups and the three groups was not significant(P=0.189>0.05),indicating that the five groups and three groups had a quick effect. At the end of 2 weeks of treatment, the NPQ scores of the three groups were similar(P>0.05),and the efficacy lasted until 1 month of follow-up,suggesting that the stability of the effect was still acceptable. At the end of 3 months of follow-up,the NPQ scores of the three groups increased substantially, and the extent of recovery was basically the same(P>0.05),suggesting that with the increase of time, the cervical pain of the three groups had recurrence, and the difference was not significant.(3)Secondary Outcome Indicators:Simple McGill Pain Scale Score(a)Intra-group comparison:After treatment,McGill scores of the three groups decreased significantly in each observation point(P<0.05),suggesting that different frequency of shallow needling with filiform needles can effectively alleviate cervical pain in patients with cervical spondylosis.(b)Inter-group comparison:After treatment,McGill scores were reduced in all three groups.Five times a week was better than two times a week in improving McGill scores(P=0.025<0.05),but not better than three times a week(P=0.168>0.05).It showed that five times a week had a quick onset and the effect was similar to that of three times a week. After 2 weeks of treatment, the McGill scores of the three groups decreased further, and the curative effect of the three groups was similar(P<0.05),and the therapeutic effect lasted for one month.During the follow-up period of 3 months, the McGill scores of the three groups showed a significant increase, and the increase was basically the same(P<0.05),indicating that the pain of cervical spondylosis in the three groups had recurrence with the increase of time, and the recurrence rate was similar.(4)Comparison of overall efficacy(a)Intra-group comparison:The overall efficacy of the three groups at the end of the treatment was significantly improved(P<0.05),suggesting that the treatment of cervical spondylosis by superficial needling with filiform needles has a definite clinical effect.(b)Intergroup comparison:At the end of the course of treatment, the total effective rate of the five groups per week was 82.35%,the total effective rate of the three groups was 88.57%,and the total effective rate of the two groups per week was 68.75%.However,there was no significant difference among the three groups(P>0.05),suggesting that the overall curative effect of the three groups was similar, but the apparent rate of the three groups per week was higher than that of the other two groups, while statistics did not show the possible reasons for the difference.Because the sample size is insufficient, the sample size can be further expanded in future research to obtain more accurate data.ConclusionThree groups of frequency of superficial needling with filiform needles for cervical spondylosis and cervical pain were significantly effective, and the curative effect was consolidated.Compared with the group of 2 times a week, the group of 5 times a week and the group of 3 times a week worked faster.Combined with health economics, the treatment frequency of three times a week is the best, which can ensure the curative effect, save medical costs and reduce the burden of patients.
