Clinical Study On Different Course Of Neck Pain Of Cervical Spondylosis Treated With Combination Of Acupuncture And Intradermal Needle

Objective:This paper aimed to evaluate the influence of acupuncture combined with intradermal needle treatment on patients with cervical spondylosis of different course and discuss the correlation between the efficacy and the course of cervieal spondylosis.Methods:The study used a prospective design with 100 eligible cases included. They were assigned to three groups and there were 33 cases in group A with course equal or less than a year; 35 cases in group B with course between a year and three years (including); and 32 cases in group C with course between three years and five years (including).All three groups took therapies of acupuncture followed by intradermal needle treatment. Patients were needled at following points:DaZhui(DU-14), JianZhongShu(SI-15)both sides, ZhongZhu(SJ-3), and EX-B2(both sides). Patients were maintained at sit position. Routine operation after disinfection, points were inserted at a depth of 10-30mm according to the location of point and the somatotype of patients. Practitioners twisted the needles with the puncturing method of rapid lifting thrusting and twirling in small amplitude to achieve "Deqi" sensation (described as numb, distended and aching). The needles were left for 20 min and then removed. Patients restd for 5 minutes after withdrawing needles, and then took intradermal needling therapy if they had no uncomfortable feeling.The treatment was totally 10 times for one course and must be completed within 4 weeks. Curative effects were evaluated by using Northwick Park (NPQ) and the Visual Analogue Scale (VAS). Results were obtained at the start of treatment, at the end of treatment and 4 weeks after the end of treatment.The original data were inputted to a database set up by Epidata3.1 and analyzed by SPSS20.0 with methods including descriptive analysis, analysis of variance, Kruskal-Wallis, and chi-square test etc. Results:100 eligible subjects were included in this research, the proportion of drop off was 26.3%, the total effective cases was 93 and each group had 31 cases finally.1. The difference in terms of gender, age, occupy, education, clinical diagnosis, tcm syndrome differentiation, NPQ scores and VAS scores before treatment among three groups was no significant (P> 0.05), indicating homogeneity and comparability among three groups.2. NPQ scalesIntra-group compari son:Compare the NPQ scores at each time point of the three groups with that of before the treatment, there were significantly decline (P=0.000).Comparision among groups:At the end of the treatment, group A (disease course≤one year) was 13.58 ±5.22; group B(one year<disease course≤three years) was 18.66±7.42; group C(three years< disease course≤five years) was 23.40±8.10. There was a significance difference between group A and group B (P=0.033), as well as group A and group C (P=0.000), respectively. Group A and group B has significance difference (P=0.049).One month after the end of the treatment, group A was 17.72±5.32, group B was 24.13±10.34, and group C was 27.15±8.82. Group A and group B had significant difference (P=0.046), as well as group A and group C (P=0.000) Group B and C has no significance difference (P=0.207).3. VAS scalesIntra-group comparison:Compare the VAS scores at each time point of the three groups with that of before the treatment, there were significantly decline (P=0.000).Comparision among groups:At the end of the treatment:goup A was 1.60±0.71, group B was 2.50± 0.80, group C was 3.47±0.84. There was a significant difference between group A and group B (P=0.002), as well as group A and group C (P=0.000). Group B and C has significance difference (P=0.002)After a month of the end of the treatment, group A was 2.20±0.60, group B was 3.84±1.05, and group C was 4.10±0.85. Group A and group B had significantly difference (P=0.000), as well as group A and group C (P=0.000) Group B and group C was statistically comparable(P=1.000).4. Correlation AnalysisCorrelative analysis showed that at the end of the treatment the course respectively negative correlated with the reducing score rate of NPQ and the decreased score of VAS (r=-0.499, P=0.000;r=-0.576, P=0.000).5. EfficiencyAt the end of the treatment the total effective rate of the three groups was 93.54%,90.32% and 87.10%, respectively. The obvious effective rate of the three group was 51.61%% 12.90% and 0, respectively. There was significant difference (x2=26.902, P=0.000).Conclusion:The therapy of Acupuncture combined with Intradermal Needle for the different courses of neck pain caused by Cervical Spondylosis has a effective clinical efficacy. The course has negative correlation with the efficacy.The treatment had better efficacy for patients with course within one year than those with three years and 3-5 years respectively, and the efficacy of three years was better than those with 3-5 years.
