Research objective:To observe the clinical effect of Yqi Guyuan acupuncture method on urinary incontinence patients after radical proststectomy of syndrome of non-consolidation of kidney Qi.To verify the clinical efficacy of acupuncture and moxibustion therapy proposed on the basis of "Yiqi Guyuan" theory of TCM,and combined with rehabilitation therapy to accelerate the recovery of post-prostatectomy urinary incontinence.To provide more practical basis for the treatment of this disease and the expansion of clinical indications of acupuncture and moxibustion.Research methods:Forty patients were divided into treatment group and control group,20 cases each.The treatment group was treated with Yiqi Guyuan acupuncture method combined with pelvic floor muscle training(PFMT),while the control group was treated with PFMT.All the enrolled patients were given the guidance of conventional behavioral therapy for urinary incontinence after radical prostatectomy.Patients in the control group received 100 times of pelvic floor muscle training every time,3 times a day,once in the morning,once in the afternoon and once in the evening,one course of treatment for 1 month,a total of 3 courses of treatment.On the basis of the treatment plan of the control group,patients in the treatment group were given Yiqi Guyuan acupuncture method,and received acupuncture treatment once every other day,3 times a week,12 times a course of treatment,a total of 3 courses of treatment.Before and after treatment,the patients in the two groups were given 72-hour pad dosage,the Chinese version of the incontinence questionnaire of the international incontinence advisory committee(ICI-Q-SF),the quality of life questionnaire for incontinence(I-QOL),TCM syndrome score as evaluation indicators to observe the efficacy.All clinical data were analyzed by SPSS 21.0 statistical software.Research results:1.Baseline condition:the age,body mass index,tumor stage and Karmofsky performance status(KPS)scores of the two groups were statistically compared before treatment,showing no significant difference(P>0.05),which was comparable.2.72-hour pad dosage:Before treatment:there was no significant difference between the two groups(P>0.05),which was comparable.After treatment:Within-group comparisons were statistically significant(P<0.01);Between-group comparisons were statistically significant(P<0.01).The results showed that after treatment,the amount of 72 hours urine pad used decreased in the two groups,and the amount of reduction in the treatment group was better than that in the control group.3.ICI-Q-SF score:Before treatment:there was no significant difference between the two groups(P>0.05),which was comparable.After treatment:Within-group comparisons were statistically significant(P<0.01);Between-group comparisons were statistically significant(P<0.05).The ICI-Q-SF scores of the two groups were improved after treatment,and the improvement of the treatment group was better than that of the control group.4.I-QOL score:Before treatment:there was no significant difference between the two groups(P>0.05),which was comparable.After treatment:Within-group comparisons were statistically significant(P<0.01);Between-group comparisons were statistically significant(P<0.01).It shows that the I-QOL scores of the two groups are improved after treatment,and the improvement of the treatment group is better than that of the control group.5.TCM syndrome score:Before treatment:there was no significant difference between the two groups(P>0.05),which was comparable.After treatment:Within-group comparisons were statistically significant(P<0.01);Between-group comparisons were statistically significant(P<0.05).After treatment,the scores of TCM syndromes in both groups were improved,and the improvement in the treatment group was better than that in the control group.6.Clinical efficacy of the two groups:In the treatment group,2 cases(10%)were cured,10 cases(50%)showed significant effect,and the cured and markedly effective rate was 60%.In the control group,there were 0 cases(0%)cured,3 cases(15%)significantly effective,and the cured and markedly effective 15%.The curative effect of the treatment group was better than that of the control group.7.Occurrence of urinary incontinence:(1)failure to reach the toilet results in urinary incontinence:before treatment(P<0.01),there were significantly more patients in the treatment group than in the control group who would have urine leakage if they failed to reach the toilet.Intra-group comparison after treatment:treatment group(P<0.01),control group(P>0.05).Comparison between the two groups after treatment:the symptom improvement rate of the treatment group was 100%,while that of the control group was 57.14%.(2)urinary incontinence during coughing or sneezing:There was no difference between the two groups before treatment,which was comparable.Intra-group comparison after treatment:treatment group(P>0.05),control group(P>0.05).Comparison between the two groups after treatment:symptom improvement rate of 10%in the treatment group and 5%in the control group,there was no significant difference between the two groups.(3)urinary incontinence during sleep:the two groups of patients before treatment(P>0.05)were comparable.Intra-group comparison after treatment:treatment group(P<0.01),control group(P>0.05).Comparison between the two groups after treatment:the symptom improvement rate of the treatment group was 88.89%,while that of the control group was 66.67%,which was different between the two groups.(4)urinary incontinence during activities or sports:patients in the two groups before treatment(P>0.05)were comparable.Intra-group comparison after treatment:treatment group(P<0.01),control group(P<0.05).Comparison between the two groups after treatment:symptom improvement rate was 40.00%in the treatment group and 31.58%in the control group,and there was no significant difference between the two groups.(5)incontinence occurred after urination and after dressing:patients in the two groups before treatment(P>0.05)were comparable.Intra-group comparison after treatment:treatment group(P<0.01),control group(P<0.01).Comparison between the two groups after treatment:symptom improvement rate was 92.31%in the treatment group and 90.00%in the control group,and there was no significant difference between the two groups.Research conclusions:The Yiqi Guyuan acupuncture method combination of PFMT and PFMT alone can improve urinary incontinence after radical prostatectomy.However,after 3 courses of treatment,the improvement of urinary incontinence symptoms in patients treated with Yiqi Guyuan acupuncture method combined with PFMT was better than that treated with PFMT alone.
