The Study Of The Construction Of Management Mode Of Chronic Disease In Community Pharmacy—take Diabetes As An Example

Objective:Under the background of pharmaceutical service mode transformation,analysis the pilot situation of Chinese pharmacist prescribing system,the system of foreign pharmacist prescribing,and the definition of pharmacist prescribing,to put forward the basic ideas and scheme design of the prescription right system of pharmacists in China and to provide theoretical basis for relevant decision-making departments to further consolidate and improve the prescription right system of pharmacists in China.Methods:Qualitative and quantitative methods were used to search,extract,statistics,analyze and summarize the research of pharmacists' prescription right system,the effectiveness evaluation of pharmacists' prescription right system and the pilot development of pharmacists prescription right system in China.By using the comparative research method,it takes the United Kingdom,Canada and the United States as the research objects,finding out the similarities and differences of the systems,and summarizing the rules of the prescription right system of pharmacists,so as to provide reference for the construction of the prescription right system of pharmacists in China.On the basis of summarizing the commonness of foreign pharmacists'prescription right system,the author compares the pharmacists' prescription right system with domestic relevant systems to find out the defects of China's existing system and ways to improve it,Results:From the quantitative research results,pharmacists with prescription power can improve patients,medication compliance and self-management ability.It can significantly improve the clinical treatment effect of patients and significantly reduce the incidence of adverse drug reactions.From the perspective of practical demand,China is in the transition period of pharmaceutical service mode,and pharmacists must transform their functions.In this process,pharmacists are inevitably involved in prescription management services.From the pilot situation,many places in China have independently explored the pharmacist prescription right system,providing rich experience reference for the establishment and improvement of pharmacist prescription right system.Conclusion:in the pilot areas of the prescription right system of pharmacists in China,the roles and responsibilities of pharmacists in all aspects of pharmaceutical services have been clarified and the level of rational drug use has been improved by granting the prescription right of pharmacists with certain authority,setting up pharmaceutical outpatient service and establishing cooperative drug treatment management system.However,in the process of pilot,the prescription right of pharmacists has always been controversial.Therefore,the prescription right of pharmacists is constructed from three aspects:the subj ect of the right,the content of the right and the conditions of obtaining the right.The system of prescription right of pharmacists in China is designed from the perspective of regulatory agencies and path selection.The system of pharmacists'prescription right can fully reflect the new connotation of government responsibility,high quality and preferential treatment and fair access.Under the situation of coordinating the family doctor system,it can help to maintain citizens'right to health and promote the realization of basic pharmaceutical services for all.,disease is formed in strengthening disease management efficiency,service timely response to the aging society for the prevention and control of sexually transmitted disease problems,ensuring the health poverty alleviation work and reduce the doctor workload,control,medical and health care cost growth highlights the necessity of policy implementation,to enhance people's confidence in the government and medical treatment are of great significance.
