Objective:1.To explore the qualitative study of traditional CT combined with MRI in the differential diagnosis of benign and malignant lacrimal epithelial tumors,to classify the significance of imaging findings,and to make a statistical analysis.2.To explore the quantitative analysis of benign and malignant differentiation of lacrimal gland epithelial tumor based on multi-parameter 3.0TMRI tumor heterogeneity analysis,evaluate the diagnostic efficacy and compare it with the qualitative research of traditional CT combined with MRI,and provide reference for clinical diagnosis and treatment.Materials and methods:1.Preoperative imaging data(CT and MRI)and clinical data of patients with lacrimal gland epithelial tumor confirmed by surgical resection in our hospital from January 2014 to February 2019,a total of 46 cases were collected.Benign pleomorphic adenoma group(28 cases)and malignant lacrimal gland epithelial tumor group(18 cases),combined with clinical data and postoperative pathology,statistically classified imaging features of benign and malignant significance.2.From January 2014 to February 2019,surgical resection was performed in our hospital.Postoperative pathology confirmed the preoperative 3.0T MRI data of patients with lacrimal gland epithelial tumors,a total of 43 cases;for axial T2 WI,T1WI,T2 pressure,The axial and coronal enhanced T1 WI sequences were segmented using ITK-SNAP software.The selected portion contains the largest lesion area in which manual segmentation of the region of interest(ROI)is performed.Under the premise of not obtaining postoperative pathology,the two attending physicians performed independent segmentation(both have more than 6 years of diagnostic experience).The ROI profile was read and processed using the Artificial Intelligence Kit(A.K,GE Pharmaceuticals)software.Extract valuable omics features.According to the postoperative pathology,the benign pleomorphic adenoma group(28 cases)and the malignant lacrimal gland epithelial tumor group(15 cases)were statistically analyzed,and the ROC curve was drawn to obtain the differential diagnosis of benign and malignant sequences.For regrouped patients,the diagnostic power of conventional CT combined with MRI plain scan + enhancement for benign and malignant identification of lacrimal gland epithelial tumors was recalculated.Results:1.Traditional CT combined with MRI is useful for the qualitative diagnosis of lacrimal gland epithelial tumors.Malignant tumors are more prone to density or signal inhomogeneity,irregular morphology,bone destruction,encapsulation of the optic nerve and involvement of the iliac crest.2.Multi-parametric MRI-based tumor heterogeneity analysis in the differential diagnosis of lacrimal gland epithelial tumors,the traditional CT combined with MRI for the diagnosis of lacrimal gland epithelial tumor AUC value(area under the curve)is 0.864;based on imaging diagnostic markers spherical disproportion The AUC values(area under the curve)of the ROC curves on the T1 WI,axial enhanced T1 WI,T2WI,T2 fatliquoring,and coronal enhanced T1 WI sequences were 0.867,0.900,0.910,0.926,and 0.721,respectively.Combined fat suppression sequence and axial T1 WI enhanced sequence,AUC value of 0.955,its diagnostic efficiency is higher.3.The benign group based on the T2 fat suppression sequence was between 1.12 and 1.43 in the benign group and 1.32-1.64 in the malignant group;and its diagnostic threshold is 1.363,the qualitative screening was difficult to diagnose in the separate study;The benign group was 1.32,which was lower than the diagnostic threshold.The malignant group was 1.40,higher than the diagnostic thresholds;for individual lacrimal epithelial tumors that cannot be identified by conventional CT combined with MRI,we can try to quantify the multiparametric MRI tumor heterogeneity.Conclusion:Traditional CT combined with MRI qualitative study and multi-parameter MRI tumor heterogeneity quantitative study are useful for the differential diagnosis of lacrimal gland epithelial tumors.The quantitative diagnosis based on multi-parametric MRI tumor heterogeneity is more effective and can be used for clinical diagnosis and treatment.And the prognosis provides a more accurate basis.
