ObjectiveThere are many different kinds of tumors in lacrimal gland area. This article summarizes the clinical features of the lacrimal gland tumors, and analysis the acoustic characteristics of different tumor imaging and color Doppler flow characteristics. Control pathological findings, research lacrimal gland tumors with color Doppler ultrasound images showed correlation between pathology, and provide help for clinical diagnosis.MethodsChoose during Jan2008to Oct2013, Tianjin eye hospital confirmed by pathological histology of72cases of lacrimal gland tumor patients. Analysis the preoperative information in patients, including sex, ages, clinical symptoms and signs. At the same time, do the color Doppler ultrasound and record the ultrasonic ultrasonographic indicators, like the lesion location, shape, border, internal echo, acoustic attenuation, volume, location, distribution, shape, and flow parameters of blood flow signals were summarized. The results were compared with pathological study of the lacrimal gland tumors characterized by color Doppler ultrasound.ResultsAmong the72patients, male29cases (40.28%), female43cases (59.72%); The average age was42.74±16.87years old, the maximum age was78years old, the minimum age was13years old, the median age was46years old. Monocular66cases (91.66%), binocular6cases (8.34%). Among the78eyes, right eye was found in34cases (43.59%), and left eye was found in44cases (56.41%); The average duration was14.99±16.09months, the longest duration was8years and the shortest was half a month. Among the72patients, exophthalmos55cases (76.39%), neoplasm was found in lachrymal area51cases (70.83%), eyelid swelling42cases (58.33%), pain21cases (29.17%), eye movements restricted38cases (52.78%), diplopia12cases (16.67%), diminution of vision19cases (26.39%), and ptosis16cases (22.22%). The most common symptoms was exophthalmos (76.39%) and lacrimal gland area neoplasm (70.83%);In the postoperative pathologic results, epithelial tumors46cases (63.89%), Non-epithelial tumors14cases (19.44%) and inflammatory pseudotumor12cases (16.67%).Among the epithelial tumors, there were32cases (44.44%) plemorphic adenoma,11cases (15.28%) adenoid cystic carcinoma and3cases (4.17%) adenocarcinoma.B-ultrasound showed lacrimal gland pleomorphic adenoma lesions was round, boundary was explicit, and the echo was uniform; Adenoid cystic carcinoma was flat shape, boundary was explicit, the echo was less and the sound attenuation of medium; Adenocarcinoma of round or irregular shape lesions, the boundary was not clear and the echo moderate; Non-Hodgkin’s lymphoma was irregular, boundary was not clear, the echo was less and sound attenuation was not significant; Inflammatory pseudotumor was irregular, boundary was clear, the echo was less and sound attenuation was significant. CDI tests showed lacrimal gland pleomorphic adenoma internal no blood flow signals or blood flow was not rich, while the other four kinds of tumors blood flow signals was rich. Measured pleomorphic adenoma PSV11.06±5.13cm/s, EDV1.94±1.17cm/s, RI0.49±0.21; adenoid cystic carcinoma PSV16.99±7.13cm/s, EDV5.75±2.11cm/s, RI0.75±0.15; Non-Hodgkin’s lymphoma PSV17.34±7.66cm/s, EDV5.47±3.19cm/s, RI0.65±0.11; inflammatory pseudotumor PSV18.66±9.86cm/s, EDV5.09±3.14cm/s, RI0.66±0.13.For lacrimal gland pleomorphic adenoma%adenoid cystic carcinoma、Non-Hodgkin’s lymphoma and inflammatory pseudotumor, significant difference was found for PSV、 EDV and RI value among the perimeters in these four kinds of tumors.ConclusionLacrimal gland tumor of color doppler ultrasound manifestations and pathological histology characteristics are closely related. So, the use of color doppler ultrasound can be roughly infer that the lacrimal gland tumor histologic types.Color doppler ultrasound has simple operation, non-invasive, repeatability, can be observed under dynamic display the characteristics of pathological changes in hemodynamic information. It can be in CT or MRI examination on the basis of qualitative diagnosis and differential diagnosis of tumor. For the diagnosis and treatment of lacrimal gland tumor has a guiding significance.
