Clinical Observation Of The Treatment Of Danzhi Xiaoyaosan Jiawei Combined With Accupuncture With Heated Needle On Liver-based Depression Fire Type Circumscribed Neurodermatitis

Objectve: Through research on the treatment of Danzhi Xiaoyaosan Jiawei combined with accupuncture with heated needle on liver-based depression fire type circumscribed neurodermatitis,skin lesion scores,efficacies,degree scores of itching,TCM symptom scores,HAMA scores,adverse reactions and recurrence rate for three groups of patients are observed before and after treatment,so as to evaluate the clinical efficacy of the treatment of Danzhi Xiaoyaosan Jiawei combined with accupuncture with heated needle on liver-based depression fire type circumscribed neurodermatitis.Exploring the advantages of this method in the treatment of liver-based depression fire type circumscribed neurodermatitis may find a more convenient,effective and safe way for the treatment of neurodermatitis.Methods: This study was conducted to observe 60 patients with liver-based depression fire type circumscribed neurodermatitis diagnosed in the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from Dec.2017 to Dec.2018,wherein the ratio of male to female is 1:1,and the age distribution range is 18-65 years old..According to the order of treatment,60 patients were divided into three groups according to the random number table method,i.e.,a combination group(Danzhi Xiaoyaosan Jiawei combined with heated needle)and a control group I(heated needle)and a control group II(Danzhi Xiaoyaosan Jiawei).The patients in each group were treated with topical paeonol ointment twice a day to record skin lesion scores,efficacies,degree scores of itching,TCM symptom scores,HAMA scores,adverse reactions and recurrence rate before treatment and after 4 and 8 weeks of treatment.Then,statistical analysis was performed on the data obtained according to statistical software(SPSS 21.0),and an objective evaluation was made on the results.Results: Before treatment,balance comparison for the general conditions of the three groups of patients,namely gender,age and course of disease,were conducted.The difference was not statistically significant(P>0.05),which was comparable.(1)Comparison of efficacies: The efficacy of the three groups of patients at the end of the fourth week of treatment was evaluated,and the results were as follows: the comined group(30%)> the heated needle group(20%)> TCM group(15%).There was no statistical difference in the efficiency of the three groups.In the 8th weekend,the combined group(95%)> the heated needle group(55%)= the TCM group(55%)wherein there was statistical difference between the combined group and the heated needle group,the combined group and the TCM group(P<0.05).There was no difference in efficacy between the heated needle group and the TCM group,indicating that the combined group had the best therapeutic effect on liver-based depression fire type circumscribed neurodermatitis.(2)Comparision of skin lesions scores: there was no difference in the skin lesion scores of the three groups before treatment(P>0.05),and they were better than those before treatment(P<0.05).At the end of the fourth week,there was no significant statistical difference between the combined group and the heated needle group.There was significant statistical difference between the two groups and the TCM group(P<0.05).The improvement of skin lesions in the combined group and the heated needle group in the fourth week was better than that of the TCM group.There was no significant difference in the improvement of skin lesions between the heated needle group and the TCM group at the 8th week(P>0.05).There was significant statistical difference between the combined group and the heated needle group,the combined group and the TCM group(P<0.05).The combined group was superior to the control groups in the improvement of skin lesions,but there was no significant difference between the control groups.(3)Comparision of degree scores of itching: the degree scores of itching in the three groups were not different before treatment(P>0.05),and were better than those before treatment(P<0.05).At the end of the fourth week,there was no significant statistical difference between the combined group and the heated needle group.There was significant statistical difference between the above two groups and the TCM group(P<0.05).The combined group and the heated needle group had better improvement on the itchning than the TCM group at the end of 4th week.There was no significant difference in the improvement of itching between the TCM group and the heated needle group at the end of the 8th week(P>0.05).There was a statistical difference between the combined group and the two control groups(P<0.05).At the 8th weekend,the combined group was superior to the control groups in the improvement of itching,but there was no significant difference between the control groups.(4)Comparison of TCM symptom scores: There was no difference in TCM syndrome scores between the three groups before treatment(P>0.05),and they were better than those before treatment(P<0.05).At the end of the 8th week,there are differences between the three groups;for improvement in the TCM syndrome,the combined group > the TCM group > the heated needle group,indicating that the combined group has the best effect in improving TCM syndrome scores.(5)Comparison of anxiety inventory scores: There was no difference in the anxiety inventory scores of the three groups before treatment(P>0.05),and they were better than those before treatment(P<0.05).There was no difference between the combined group and the TCM group.There was a difference between the above two groups and the heated needle group,indicating that the combined group and the TCM group had better improvement of anxiety than the heated needle group.(6)The occurrence of adverse reactions: 1 case in each of the combined group,the heated needle group and the TCM group.(7)For the interview after the end of the treatment,the recurrence rate of the combined group was lower than that of the TCM group.Conclusion: The research of this subject shows that Danzhi Xiaoyaosan Jiawei combined with accupuncture with heated needle on is superior to monotherapy in the treatment of liver-based depression fire type circumscribed neurodermatitis in terms of skin lesion changes,efficacies,itching relieving,and improvement of anxiety.Its clinical efficacy is high,safety is good,and the recurrence rate is low.So,this is a good and effective method for the treatment of liver-based depression fire type circumscribed neurodermatitis.
