Effect Of Yiqi Jiedu Huoxue Method On PSP,APN And Blood Lactic In Patients With Hospital Acquired Pneumonia

1.Objective: The main purpose of this study is to observe the changes of PSP,APN,blood lactic acid and related indicators before and after treatment in patients with hospital-acquired pneumonia treated with Shenmai Injection + Chonglian Oral Liquid + Salvia Miltiorrhiza Polyphenolate.2.Method:From March 2018 to December 2018,63 patients with hospital acquired pneumonia were admitted to the Department of Severe Medicine(MICU and SICU)of Anhui Hospital of Traditional Chinese Medicine.According to the time of admission,in the way of lottery patients were divided into two groups(31 cases in the matched group,distinguish 32 cases in the treatment group).The matched group was treated with western medicine treatment(active treatment of primary diseases;anti-infection,phlegm resolving,strengthening respiratory tract govern;resistence and treatment of stress ulcer;maintain water and electrolyte balance;prevent deep venous thrombosis;nutritional support,etc.).The treatment group was added Shenmai Injection + Chonglian Oral Liquid + Salvianolate on the basis of the matched group for 7 days.The changes of PSP,APN,blood lactate,Pa O2,Pa CO2,oxygenation index(OI),white blood cell count(WBC),procalcitonin(PCT),hypersensitive C-reactive protein(hs-CRP),clinical pulmonary infection score(CPIS score),acute physiology and chronic health score II(APACHE-II score)and TCM symptoms before and after treatment were observed and compared.Through statistical analysis of the relevant data,the conclusion is drawn and judged that there is no statistical significance,and ultimately determine the treatment effect.At the same time,probe into the possible mechanism-processed channel,for integrated Chinese and Western medicine cure of hospital acquired pneumonia to explore effective remedy ideas and methods.3.Result:(1)Observation of PSP changes,after 7 days of treatment,the values of PSP in the treatment group were lower than those in the matched group,and the difference between the two groups was significant(P<0.05).After 7 days of treatment,APN in the treatment group was significantly higher than that in the control group,and the difference between the two groups was statistically significant(P<0.05).(2)About the blood lactic acid,both of the groups were reduced after treatment,and the difference between the two groups was statistically significant(P<0.05).(3)Pa O2,Pa CO2 and OI were observed.About the level of Pa O2?OI,the two groups were all higher than before and the level of Pa CO2 were lower than before after treatment,the increase of Pa O2?OI and the decrease of Pa CO2 in the treatment group was more obvious,and the difference between the two groups was statistically significant(P<0.05).(4)In terms of WBC,PCT and CRP,after 7 days of treatment,the values of both groups were lower than before,and the level of treatment group decreased more significantly than that of matched group.the difference between the two groups was statistically significant(P<0.05).(5)Yiqi Jiedu Huoxue method can significantly reduce the CPIS score,APACHE-II score and TCM symptom score of patients with hospital-acquired pneumonia,alleviate the symptoms of patients,and the clinical effect is obvious.(6)Observing prognosis of disease,it was obvious that the improved in the treatment group was significant better than the matched group after 7 days treatment.The difference is meaningful through statistical test(P<0.05).(7)Both of the groups had no obvious adverse reactions during the treatment,it means that the treatment method was safe and reliable.4 Conclusion:(1)The method of invigorating qi,detoxifying and activating blood circulation can effectively reduce the level of PSP in patients with hospital acquired pneumonia,suggesting that it can effectively inhibit the further development of inflammatory reaction,reduce the damage of inflammatory factors to the body,and reduce the effect of pulmonary infection.At the same time,it can effectively enhance the level of APN in patients and inhibit the excessive progress of systemic inflammatory response,,suggesting that APN can enhance the body's resistance to pulmonary infection.(2)Yiqi Jiedu Huoxue method can effectively reduce blood lactic acid in patients with hospital acquired pneumonia,suggesting that it can improve organ perfusion oxygenation,correct hypoxic internal environment and promote the body circulation.(3)The method of invigorating qi,detoxifying and activating blood circulation can effectively increase PO2 and Fi O2 of patients with hospital acquired pneumonia and decrease PCO2,suggesting that it can promote pulmonary ventilation,ventilation function and improve body oxygenation.(4)Yiqi Jiedu Huoxue method can reduce WBC,PCT and CRP values of inflammatory factors in patients with hospital-acquired pneumonia,suggesting that this method can enhance the anti-inflammatory response of the body and effectively alleviate pulmonary infection.(5)Yiqi Jiedu Huoxue method can significantly reduce APACHE-II score and TCM symptom score of patients with hospital-acquired pneumonia,and promote the alleviation of clinical symptoms.(6)The mechanism of Yiqi Jiedu Huoxue method may be related to enhancing the systemic immune response,antagonizing the progress of inflammatory response,reducing the over expression of inflammatory factors,improving systemic circulation perfusion,protecting lung function,and so on,and ultimately improving the progress and prognosis of patients.
