Objective:Through evaluate the application value of thermostatic amplification microarray method in pathogen detection of severe hospital-acquired pneumonia.To understand the pathogenic bacteriological characteristics of acquired pneumonia in intensive care hospitals.Methods:1.patients who were admitted to the department of critical care medicine of affiliated hospital of xx university from June 2018 to June 2019 and met the diagnosis of hospital-acquired pneumonia were collected.Samples of the lower respiratory tract were taken within 1 day after diagnosis.2.The results of hospital bacteria detection and drug resistance in the lower respiratory tract of pneumonia acquired in intensive care hospital of affiliated hospital of xx university from 2017 to 2019 were collected..Results: 1.During the study period to participate in a total of 100 patients with detection,severe hospital acquired pneumonia in patients with lower respiratory tract pathogen test results are as follows: thermostatic expansion chip test positive for pathogenic bacteria of 83 cases,54 cases of mixed pathogen infection,were detected from 188 strains,the major detected 69 strains of acinetobacter baumannii strains,methicillin-resistant staphylococcus aureus 31 strains,26 strains with klebsiella pneumoniae.There were 79 cases of pathogen positive in bacterial culture and 27 cases of mixed pathogen infection.A total of 97 strains of pathogen were detected.The main bacteria detected were 59 strains of acinetobacter baumannii,16 strains of pseudomonas aeruginosa and 10 strains of klebsiella pneumoniae.The positive detection rate of acinetobacter baumannii,klebsiella pneumoniae and staphylococcus aureus was better than that of bacterial culture,but the positive detection rate of pseudomonas aeruginosa was worse than that of bacterial culture.2.2017 years-2019 severe lower respiratory tract in hospital acquired pneumonia pathogen detection results: a total of 361 strains pathogenic bacteria detection,312 strains of gram-negative bacteria,accounting for 86.42%,among them with acinetobacter baumannii(154 strains,42.56%),pseudomonas aeruginosa(68 strains,18.83%),with klebsiella pneumoniae(52 strains,14.40%),gram positive bacteria 30 strains,accounting for 8.31%,with staphylococcus aureus(28 cases),7.75%,main,fungi,with white candida(9 strains,2.49%).Pathogenic bacteria to drugs with high percentages of situation is as follows: acinetobacter baumannii to ceftriaxone,cefotaxime,gentamicin resistant rate of 81.81%,81.16%,79.87%,pseudomonas aeruginosa to cefotaxime,ceftriaxone,imine culture south resistant rate of 44.92%,30.43%,23.18%,pneumonia klebsiella rod bacteria cefotaxime and cefepime resistant rate of 100%,96.15%,staphylococcus aureus to penicillin G,ampicillin,erythromycin resistant rate of 92.85%,85.71% and 71.42%,respectively.Conclusion:1.The constant temperature amplification microarray method provides a more simple,sensitive and effective method for the diagnosis of pathogens in the lower respiratory tract of patients with severe hospital acquired pneumonia.2.Gram-negative bacteria were the main pathogens in the lower respiratory tract of severe hospital-acquired pneumonia,and the main infectious bacteria included acinetobacter baumannii,pseudomonas aeruginosa and klebsiella pneumoniae.
