Objective:According the research of anxiety, depression of type 2 diabetic nephropathy(DN)patients, discuss the female patients with type II diabetic nephropathy produce the incidence of anxiety and depression status and influencing factors of the occurrence and the degree of anxiety depression occur relation with the severity of type II diabetic nephropathy, for the prevention and treatment of women with type II diabetes kidney disease comorbid anxiety depression status to provide clinical basis.Methods:80 female patients with type ? diabetes admitted to the endocrinology department of jiujiang first people's hospital from January 2018 to December 2018 were selected as study subjects, including 40 patients with combined nephropathy and 40 patients without combined nephropathy. At the same time,30 healthy subjects were selected as the control group.First, general data of the selected subjects were collected at admission, including age, course of disease, body mass index, and family history of diabetes. At the same time, relevant clinical test indicators of the patients were recorded(including fasting blood glucose,2-h postprandial blood glucose, glycosylated hemoglobin(HbA 1 c),urea nitrogen,creatinine,c-peptide insulin,microalbumin,-microglobulin,diabetic nephropathy, diabetic foot and diabetic retinopathy). The subjects were scored by the anxiety self-rating scale(SAS)and the depression scale(SDS),and their basic information was statistically described.Diabetic nephropathy was divided into five stages according to the glomerular filtration rate(GFR)and urinary microalbumin excretion rate(UAE).The subjects were scored by the anxiety self-rating scale(SAS)and the depression scale(SDS),and their basic information was statistically described.Diabetic nephropathy was divided into five stages according to the glomerular filtration rate(GFR)and urinary microalbumin excretion rate(UAEComputing research group the incidence of anxiety depression, analysis of female patients with type ? diabetic nephropathy anxiety depression and personal information, human body parameters, the relationship between clinical laboratory index,in order to investigate risk factors related to anxiety depression occur,at the same time analysis the degree of anxiety depression occur relationship with the severity of type 2 diabetic nephropathy.Results:1.Comparison of general data and clinical test indicators between the DN group and the non-dn group showed that there were statistically significant differences in age, course of disease.HbAlc,urinary microalbumin, and b-microglobulin(p<0.05).The risk of diabetic foot and diabetic retinopathy in DN patients was significantly higher than that in the non-dn group(p<0.05).2.The mean levels of SAS and SDS in diabetic nephropathy patients were(46.23 11.75)and(51.13 12.46),respectively,which were significantly higher than those in non-diabetic nephropathy patients(42.27 10.31)and healthy controls(36.578.35),with statistically significant differences(P<0.01).However,the mean levels of SAS and SDS in patients with non-diabetic nephropathy were significantly higher than those in healthy controls(P<0.01).3.Among the DN group, the average SAS score was(46.23 11.75),and the>40 anxiety index was 12,accounting for 30%of the DN group, including 7 cases of mild anxiety,4 cases of moderate anxiety and 1 case of severe anxiety. The average SDS score was(48.13 12.46),and the depression index>50 was 15 cases, accounting for 37.50%of the DN group, including 9 cases of mild depression,5 cases of moderate anxiety and 1 case of severe anxiety.4.Whether Pearson correlation analysis showed that patients with anxiety and its age, family history of diabetes, diabetes duration,BMI,and so on general data and serum FPG,2 HPG,HbA1c,urea nitrogen, creatinine, beta globulin, urine trace albumin and other laboratory test, and presence of diabetic foot, history of complications such as diabetic retinopathy were positively correlated(P<0.05),indicating that these variables are women ?)in patients with diabetes nephropathy risk factors of anxiety disorders.5.Correlation analysis showed that depression was positively correlated with the course of disease, family history of diabetes, urea nitrogen, creatinine, urinary microalbumin, presence or absence of diabetic foot and diabetic retinopathy(P<0.05),indicating that the above variables were risk factors for depression in women with type ? diabetic nephropathy.6.Rank-correlation analysis showed that the stage of type ? diabetic nephropathy in women was significantly positively correlated with the severity of anxiety and depression disorders. Conclusion:Anxiety and depression symptoms are common in type 2 diabetes mellitus and affect the quality of life of patients with the risk of complications. Therefore, early assessment of the psychological status of patients with type 2 diabetes, timely detection of adverse psychological status, analysis of its potential risks, and timely treatment can effectively control blood glucose and symptoms,as well as the screening of adverse complications in type 2 diabetes patients and better improve the prognosis have significant clinical significance.
