Objective:By investigating the quality of life of 76 patients with stage 3-4 chronic kidney disease,the present situation of quality of life of stage 3-4 patients with chronic kidney disease and the factors affecting their quality of life were summarized.Meanwhile,intervention follow-up was conducted for patients who joined chronic disease management for a period of 8 months by means of chronic disease management,and the effects of chronic disease management model on the quality of life of patients with stage 3-4 chronic kidney disease and related laboratory indexes of nephropathy were summarized,so as to provide a reference for clinical work and delay the time of patients with stage 3-4 chronic kidney disease entering end-stage renal disease.Methods:The survey by grant SF-36 scale,to patients and patients of purpose and meaning of questionnaires,fill out the method of questionnaire,on the basis of the patients to fully understand and comply with,please fill in the questionnaire,patients with the income data by SPSS statistical software of medicine and related calculation formula,calculates the patients with chronic kidney disease(CKD)3-4 SF-36 scale scores of each dimension.Some patients were included in the chronic disease management,and the chronic disease management program and follow-up program were developed for the patients who joined the chronic disease management program.The intervention follow-up was conducted for 8 months,and the differences of life quality and nephrology-related laboratory indexes between the patients who joined the chronic disease management program and the patients who did not join the chronic disease management program were compared for 8 months.Results:In this study,a total of 80 patients with stage 3-4 chronic kidney disease were included.All patients filled in the sf-36 scale,and 2 patients with incomplete data were removed.2 patients were admitted to hemodialysis within one month after enrollment.In terms of the overall quality of life of the patients,the highest score was general health(65.00±12.91)and the lowest was social function(46.21±16.75).Patients with stage 3-4chronic kidney disease showed significant differences in total scores and all dimensions--physiological function,physiological function,general health,energy and mental health(P<0.01),which showed that stage 3 patients were significantly higher than stage 4 patients.38 patients were randomly selected for chronic disease management,and 4 patients were included in the control group due to disagreeing with follow-up.Among the 34 patients included in chronic disease management,the total score of life quality,physiological function,physiological function,social function and emotional function of 34 patients were significantly improved after chronic disease management.There was no significant difference in urea nitrogen and hemoglobin in 34 patients(P>0.05).There was a significant decrease in creatinine and a rise in GFR.Systolic blood pressure and diastolic blood pressure were better than before.Compared with the chronic disease managers without chronic disease managers,there were significant differences in the total score of life quality,physiological function,physiological function,social function,emotional function and mental health between the two groups(P<0.05),which showed that the control group was significantly lower than the management group.There were no significant differences in urea nitrogen and hemoglobin between the two groups(P>0.05),and significant differences in creatinine value,systolic blood pressure,diastolic blood pressure,and GFR(P < 0.01),which showed that the creatinine value,systolic blood pressure,and diastolic blood pressure in the management group were significantly lower than those in the control group.GFR was higher in the management group than in the control group.Conclusion: The quality of life of patients with stage 3-4 chronic kidney disease is affected by many factors,especially psychological factors.The quality of life of patients with stage 3 chronic kidney disease is higher than that of patients with stage 4 chronic kidney disease.Chronic disease management means can effectively control the urea nitrogen,hemoglobin and other nephropathic indicators of patients with chronic kidney disease stage3-4,and can also effectively regulate the blood pressure of patients.It is worth popularizing clinically.
