ObjectiveIn recent years,with the increasing number of patients with chronic diseases,chronic disease management has become a key concern of the national government,but also become a worldwide problem.Some third-grade class-Ahospital in guangzhou has a large number of outpatients with chronic diseases.The diagnosis and treatment of chronic diseases demands exceeded supply.Therefore,the third-grade class-A hospital in Guangzhou has set up a special chronic disease management center to provide targeted chronic disease management services for the majority of patients with chronic disease,thus effectively improving the efficiency of chronic disease management and..reducing the burden and pressure on patients.However,there are many kinds of chronic diseases,and the severity of the disease is different,which has a serious impact on the management of chronic diseases.Therefore,it is necessary to constantly improve the chronic disease management plan and model,and improve the management effect,management quality and management satisfaction.In this study,a randomized controlled experiment of the same period was used to analyze and evaluate the management effect and management satisfaction of chronic diseases in the third-grade class-A hospital in Guangzhou so as to provide certain scientific basis and management guidance for improving the clinical application of chronic diseases management in it.MethodsIn this study,500 patients with chronic diseases were randomly selected from the third-grade class-A hospital in Guangzhou,including 100 patients with diabetes,hypertension,coronary heart disease,copd and stroke,respectively.In the same period of randomized control experiment,patients with each type of chronic disease were randomly divided into control group and management group according to their own wishes.In the control group,chronie diseases were treated and managed in accordance with the normal outpatient visits,while in the management group,the chronic diseases management program was adopted in the third-grade class-A hospital in Guangzhou,and the management time was three months.The quality of life scale was used to evaluate and analyze the total scores,physiological function,social function,psychological function and specific modules before and after the control group,the management group,between the two groups before and after management.At the same time,a questionnaire on the satisfaction degree of chronic disease management in Guangzhou was used to investigate patients involved in chronic disease management,and the satisfaction degree of chronic disease management in the hospital was obtained.T and ?2 tests were used for statistical analysis of all the data obtained.Resu!ts1.After management intervention,compared with the control group,hypertensive patients showed changes in social function in the management group(P<0.05),but no significant changes in total life quality score,physical function,psychological function and specific module scores(P>0.05).Before and after the control group:there were changes in social function,and the differences were statistically significant(P<0.05),while there were no significant changes in the rest,and the differences were not statistically significant.Patients with hypertension are more than 80%satisfied with overall sensation,screening,risk prediction,diet,exercise,psychological management and other aspects,while 90.7%are satisfied with screening,and the satisfaction of the remaining seven dimensions is more than 70%.2.After the management intervention,compared with the control group,the social function of the management group changed(P<0.05),while the total score of life quality,physical function,psychol ogical function and specific module did not change significantly(P>0.05).Before and after the control group:there were changes in the total scores of social function and quality of life in the management group,and the differences were statistically significant(P<0.05),while there were no significant changes in the rest,and the differences were not statistically significant.Through the satisfaction survey,it was found that the management group was satisfied with the overall feeling,medication compliance management,diet,exercise and psychological satisfaction of the management intervention program,while the remaining satisfaction was below 80%.3.After management intervention,compared with the control group,CHD patients in the management group showed changes in social function(P<0.05),but no significant changes in total life quality score,physical function,psychological function and specific module scores(P>0.05).Before and after the control group:there were changes in the total scores of social function and quality of life in the management group,and the differences were statistically significant(P<0.05),while there were no significant changes in the rest,and the differences were not statistically significant.Through the satisfaction survey,it was found that the management group was satisfied with the overall feeling,screening and risk prediction of the management intervention program,which were all over 80%,while the satisfaction was poor in the effect evaluation,chronic disease self-management and staff service,which were 63.6%,68.2%and 68.2%,respectively.4.After management intervention,compared with the control group,psychological function and social function of patients with copd changed in the management group(P<0.05),while the total life quality score,body function and specific module scores did not change signifieantly(P>0.05).Before and after the control group:there were changes in psychological function,social function and total score of quality of life in the management group,and the differences were statistically significant(P<0.05),while there were no significant changes in the rest,and the differences were not statistically significant.Through the satisfaction survey,it was found that the degree of satisfaction in the health education of copd was better,reaching 81%,while the degree of satisfaction in the aspects of risk stratification,copd self-management mode and medication compliance management was lower,all lower than 70%.5.After management intervention,compared with the control group,the total scores of psychological function,social function and quality of life in the management group showed changes(P<0.05),while the scores of physical function and specific modules showed no significant changes(P>0.05).Before and after the control group:there were changes in psychological function,social function and total score of quality of life in the management group,and the differences were statistically significant(P<0.05),while there were no significant changes in the rest,and the differences were not statistically significant.Through the satisfaction survey,we found that the overall feeling,di'et,exercise and psychological management satisfaction is good,the satisfaction is more than 90%,but the staff service attitude,chronic disease self-management model satisfaction is less than 80%.ConclusionChronic disease patients suffer from diseases for a long time,and suffer serious effects in physiological function,social function and psychological function.In addition,the treatment cycle of chronic diseases is relatively long,requiring long-term medication,and the compliance of patients with medication is also tested to some extent.After continuous development and improvement,the third-grade class-A hospital in Guangzhou has put forward a set of systematic chronic disease management plan,which provides a good management for a large number of patients with chronic disease.According to the investigation and research in this paper,it was found that after 3 months of management,the chronic disease management program of a third-grade class-A hospital in Guangzhou affected the quality of life of patients to some extent and improved some functions(such as physical or psychological),but there was no significant change in the physical function and specific modules,and the overall improvement was not obvious.However,according to the satisfaction survey,the satisfaction degree of chronic disease management in a third-grade class-A hospital in Guangzhou is relatively general.It is still necessary to adjust the management plan according to different types of chronic diseases,improve the management satisfaction of chronic disease patients,explore scientific evaluation methods of chronic disease management quality,and constantly optimize the management of chronic disease.
