Objective The main purpose of this study was to investigate the quality of life of patients with chronic kidney disease(CKD),to explore the protective effect of hydrogen inhalation on renal function and the improvement of quality of life in patients with chronic kidney disease,and the mechanism by which it works.Methods In the study of the quality of life of CKD patients,the Pittsburgh Sleep Quality Index Scale and the European Five-Dimensional Health Scale were used to conduct a questionnaire survey on CKD patients?We collect basic information and clinical laboratory test results of the respondents,and estimate the overall prevalence of sleep disorders in non-dialysis CKD stage 3-5 patients.The univariate and multivariate Logistic regression analysis was used to evaluate the risk factors of sleep disturbance in patients with CKD.In the part of the study on the effect of hydrogen inhalation on renal function and quality of life in CKD patients,randomized,parallel,controlled,open clinical trials will be used.The CKD patients who were admitted to the Department of Nephrology in our hospital from January 2018 to December 2019 were randomly assigned to the test group and the control group.Patients in the experimental group inhaled hydrogen and were given basic treatment as planned,the inhalation flow rate was 300 ml /min,the time was 6h/day,and it was maintained for 7 days.Patients in the control group were only given corresponding basic treatment.The basic information and clinical data of the patients were collected,and the levels of renal function changes,quality of life related scale scores,and laboratory indicators in the experimental group and the control group were analyzed.In the study of related mechanisms,oxidative stress and inflammation indicators in blood and urine were detected in two groups of patients.TNF-a,IL-6 and 8-0HDG were detected by Elisa method,SOD was detected by WST-1 method,and colorimetry was traced.Method to detect GSH-PX.Results In the study of the quality of life of CKD patients,the PSQI scores and the European five-dimensional scale VAS scores of 104 CKD patients were counted.There were 66 males(63.5%)and 38 females(36.5%).The average age was 60.30 ± 13.15 years old.CKD stage 3,4,and 5 patients were 52(50.0%),38(36.5%),and 14(13.5%).Further comparison of patients with sleep disorders(PSQI greater than 5 points)and those without sleep disorders(PSQI less than or equal to 5 points)showed that the overall sleep quality of patients with CKD was poor,with 61 patients(58.7%)having sleep disorders.There were significant differences in age,smoking history,estimated glomerular filtration rate(e GFR),Cystatin C(Cys C),PSQI score,and VAS score between the two groups(P <0.05).Univariate Logistic regression analysis revealed statistically significant differences in age,e GFR,and VAS scores(P <0.05).These factors were included in the multivariate logistic regression analysis,and it was finally determined that age was an independent risk factor for sleep disorders in patients with CKD,and the VAS score was a protective factor.Both are effective methods for assessing whether sleep disorders occur in patients with chronic kidney disease.In the part of the study of the effects of hydrogen inhalation on renal function and quality of life in CKD patients,the final results show that compared with the control group,the renal function indicators(Scr,e GFR,BUN,Cys C)and quality of life scale of the experimental group patients after hydrogen inhalation treatment The scores(PSQI,VAS)were improved to varying degrees,which verified the effect of hydrogen on the protection of renal function and quality of life in patients with CKD.In the related mechanism research,it was found that hydrogen inhalation can increase the activity of SOD and GSH-PX in the serum and urine of CKD patients,reduce the levels of 8-OHDG in serum and urine,and reduce the expression of serum TNF-a and IL-6.Thereby improving CKD oxidative stress and micro-inflammatory status.Provides an effective explanation for the mechanism of hydrogen inhalation to improve kidney function and sleep quality of life in CKD patients in the second part of the study: hydrogen inhalation can play a role in protecting renal function and improving quality of life by improving oxidative stress and micro-inflammatory status in CKD patients RoleConclusion The overall sleep quality of CKD patients is poor.Age is an independent risk factor for sleep disorders in CKD patients,while VAS score is a protective factor.Both are effective ways to predict whether sleep disorders occur in patients with chronic kidney disease.Hydrogen inhalation has protective effects on renal function in patients with CKD,and can improve their sleep quality and quality of life.Hydrogen inhalation can protect renal function and improve quality of life by improving oxidative stress and micro-inflammatory status in patients with CKD.
