China has successively established the Basic Medical Insurance System for Urban Employees,the New Rural Co-operative Medical Insurance System and the Basic Medical Insurance System for Urban Residents since 1998 to initially build a basic medical insurance system that is suitable for China's national conditions for all people.With the development of economy and the progress of society,China's urbanization process accelerates constantly.A basic medical insurance system for urban and rural residents,which is built based on the Basic Medical Insurance System for Urban Residents and the New Rural Co-operative Medical Insurance System,has relied on the financial support of the government.The problem of waste of resources in this divided management system becomes increasingly obvious.The establishment of an integrated basic medical insurance system for both urban and rural residents will contribute to enhancing the fairness of medical insurance services and put forward more scientific and reasonable requirements for China's fiscal expenditure structure arrangements.This paper consists of six parts.The first part is the introduction,which mainly introduces the background and significance of the paper,overseas and domestic research statue,and the main contents,methods,innovations and shortcomings of the research.Relevant concept definition and theoretical basis of China's financial support for the basic medical insurance system for urban and rural residents is introduced in the second part,including insurance theory,efficiency theory,public goods theory and public finance theory.In the third part,the current situation of problems in China's fiscal expenditure in terms of basic medical insurance system for urban and rural residents is introduced and analyzed.This part mainly analyzes the development history and fiscal expenditure scale and structural arrangements of the Basic Medical Insurance System for Urban Residents,the New Rural Co-operative Medical Insurance System and the basic medical insurance system for urban and rural residents that is in the process of integration to point out the existing problems.The fourth part is about the empirical analysis.First of all,the DEA method,an empirical analysis method adopted in this paper,is briefly introduced.And then,appropriate input and output indicators are selected according to the principle of indicator selection and the actual situation.At last,the Max DEA software and appropriate data are used for calculation,in order to analyze the financial input and output efficiency of China's basic medical insurance system for urban and rural residents and discover the existing problems.Experience of international basic medical insurance systems for residents is used for reference in the fifth part.In this part,the institutional frameworks and corresponding fiscal arrangements of the national basic medical insurance model of the United Kingdom,the social medical insurance model of Germany and the commercial medical insurance model of the United States are introduced to provide inspiration and reference for the development of the basic medical insurance system for urban and rural residents in China.Suggestions and conclusions are put forward in the sixth part.In this part,appropriate suggestions are put forward on accelerating the integration progress,improving the performance of fiscal subsidies,optimizing the responsibility sharing model among governments,balancing fundraising responsibilities and enhancing legislation.The main contents and viewpoints of this paper are also summarized in this part to point out shortcomings of the research.
