With the improvement of people's living standards, the health of the foot has gradually been concerned by people. The high incidence of foot disease has seriously affected people's daily life and work.As a more common foot disease, flat foot account for a large proportion of patients with foot disease. The most common treatment used as a non-surgical treatment are orthopedic insoles or orthopedic shoes. However, there are many limitations in the flat foot insoles or orthotic shoes currently on the market, because they are not customized according to the specific conditions of each patient.A few custom insoles are only made according to the three-dimensional structure of the plantar, but those insoles can not regulate the distribution of the planter pressure. Objective:the article is to analyze the difference of the foot pressure and the footprint characteristics between patients with stage ? adult acquired flat foot and normal people, and to customize the orthopedic insole for patients based on these data measured above, and observe the therapeutic effect after 3 month treatment and changes in plantar pressure and footprint characteristics.Subjects:24 normal adults and 24 stage ? adult acquired flat feet patients(two sides),48 feet in each group, each group had no significant difference in gender, age, height, weight and BMI index. The experimental data is collected by using the Italian Diasu Foot Stress Analysis and Evaluation System and the 3D Footprint System(3DPODS).Statistical methods:two-sample t-test is used between the normal group and the stage ? adult acquired group. The data of the pretherapy and post-treatment of the stage ? adult acquired flat foot are analyzed by paired t test, and the comparison between the count data groups is performed by ?2 test. The significance level of all statistical results is set to P<0.05.Result:(1)The medial arch of the stage II adult-acquired flat foot is about 1 cm lower than that of the normal person, the arch index is about 6%higher than that of the normal person, and the The average AFOAS score is 79.54.The maximum pressure, the average pressure, the midfoot load are higher than normal.the forefoot load are lower, the rearfoot load are the same as normal people. The foot angle is 7.5 degrees smaller than the average person, and the long plantar angle and the transversal angle are the same as normal people. The step width and the interstep length are not different from those of the normal person, and the stride length and the step length are significantly smaller than those of the normal person. There is no difference between meam time of stride, time of stance, time of stance DS,and time of swing, but the velocity, acceleration and deceleration are significantly smaller than normal. The patient's load variation and rearfoot/forefoot load variation were larger than normal, and the resultant of forc es(COF)is smaller than normal. The center of gravity of the patient was more lateral to the outside during walking, and the front arch is also likely to collapse.(2)After the orthopedic insole treatment, the medial arch of the patient increased by about 1.4 mm,the arch index decreased by 3%,and the AFOAS score increased by about 8 points. The maximum pressure, the average pressure, the midfoot load are all reduced compared with pretherapy. The forefoot load is increased,The foot angle is increasing to about 6.3 degrees. The interstep length is slightly reduced after treatment.volecity, acceleration and deceleration increase after treatment. The load variation and rearfoot/forefoot load variation are reduced, and the resultant of forces(COF)is increased. However, the increase of the interior arch in the sharing of the foot load is not obvious.Conclusion:1.Patients with stage ? adult acquired flat foot have increased pressure in various areas of the plantar due to the collapse of the arch,2.The stability of the foot joint is reduced, the propulsive force is weakened, the center of gravity moves to the outside, and the front arch collapse.3.After the orthopedic insole treatment, the pressure of the plantar and related joints of the patient is relieved, and the collapsed arch and valgus hind foot have certainly corrected, which increases the transformation efficiency of the force during walking, but the redistribution of the internal and external pressure is not obvious.
