OBJECTIVE:To observe the efficacy of custom orthopedic insoles for the treatment of diabetic foot by comparative analysis.METHODS:Forty-eight patients with diabetes mellitus who had diabetes due to foot pain in Changzhou Chinese Medicine Hospital from October to February 2018,including 22 males and 26 females,were selected.The study subjects were set to one person and one foot.When the patient developed bilateral foot pain,the severe side was used as the research object.After the diagnosis of diabetic peripheral neuropathy,26 patients with diabetes mellitus(DM group)and 22 patients with diabetic peripheral neuropathy(DPN group)were enrolled.20 healthy subjects from our hospital were included in the control group.The VAS score and the AOFAS score of the DM group and the DPN group were used to observe whether there was a difference in the diabetic foot in the normal person.At the same time,the DM group and the DPN group were subjected to the VAS score,AOFAS score,peak pressure,average pressure,toe load,foot axis angle,step size,pace,station time,swing time,load before and after wearing the custom orthopedic insole for three months.Changes,post/forefoot load changes.and cohesion(COF)were compared to observe the effect of custom orthopedics on diabetic foot.RESULTS:The VAS score and AOFAS score of DM group and DPN group were abnormal compared with normal people,P<0.05.The VAS and AOFAS scores of DM group and DPN group were significantly different after wearing custom orthopedic insoles,P<0.05;in the DM group and the DPN group,after wearing the custom orthopedic insole,the peak pressure of the sole,the toe load and the angle of the foot axis were significantly improved,P<0.05.The mean pressure on the soles of the feet did not change significantly,P>0.05.After wearing the custom orthopedic insoles,the DM group and the DPN group did not show significant changes in steps,pace,station time and swing time.P>0.05;DM group showed significant improvement in post/forefoot load change and combined force(COF)data after wearing custom orthopedic insole,P<0.05;and showed significant change in load change,P>0.05;DPN group showed a significant improvement in the combined force(COF)data after wearing a custom orthopedic insole,P<0.05;and the change in load and post/forefoot load changes did not show significant changes,P>0.05.Conclusion:The pain and function of diabetic patients are abnormal compared with normal people.Custom orthopedic insole has certain therapeutic effect on pain and function improvement of diabetic patients in different course of disease;custom orthopedic insole effectively realizes the stress of scattered concentration and restores the foot.The purpose of the shaft angle;although the orthopedic insole does not effectively correct the patien t's gait in the short term,its biome chanical performance is significantly corrected.To some extent,the effectiveness of orthopedic insoles is indicated.
