Investigation Research On The Situation Of Patient Safety Culture Of Community Hospitals In Shaoxing

Objective:This investigation intends to show the current Patient Safety Culture Condition of Community Hospitals(PSCCH)in Shaoxing area of China,as well as to reveal the main influencing factors of it,so as to provide reliable theoretical basis and statistical support for the intervention and improvement of the hospital safety culture condition.In this way,we want to enhance the security culture level of Shaoxing community hospitals effectively.Meanwhile,this investigation also has reference meaning for other regions in promoting hospital safety-culture condition hopefully.Method:The cross-sectional survey method was used in this investigation by conducting a questionnaire survey of the staff of six community hospitals in Shaoxing.The questionnaire used consisted of a general questionnaire and a "the patient safety cultu:re questionnaire of medical institutions" which was developed by Biyu Xu etc.of Sun Yat-sen University.The data was entered into the Epidata3.1 by two peoples,and statistical analysis was conducted by IBM SPSS 19.0.Analysis method such as general descriptive analysis,analysis of variance(ANOVA),and invariance analysis etc.was taken to reveal the difference and explain the phenomenon.Results:1.A total of six community hospitals in Shaoxing were investigated,470 questionnaires were distributed,and 437 questionnaires were returned.Among them,there were 426 valid questionnaires;the questionnaires effective recollecting rate was 90.64%.2.The overall score of community hospital patient safety culture was 3.56±0.40,and the eight influencing factors with mean scores from high-to-low orders were governor superiority(3.93±0.49),team atmosphere(3.82±0.44),communication&cooperation(3.73±0.46),safety resources(3.69±0.63),unsafe behavior(3.66±0.79),fear of humiliation(3.63±0.78),psychological safety(3.53±0.55),and proactive report(2.53±1.32).3.The survey results indicated that the main factors affecting the overall level of patient safety culture in Shaoxing are the communication with patients,education,working form,specialty etc.of community workers(including medical workers).The overall safety culture score(3.22±0.34)of those who were in frequent contact with patients was lower than that of the occasional/rare contact with patients(3.30±0.35),and the difference was statistically significant(p<0.05).What's more,for the factors of proactive reporting and fear of humliation,the scores of frequent contact with patients(2.41±1.30,2.31 ±0.79)were also lower than those of occasional/rare contact with patients(2.89±1.32,2.55±0.75).However,in the psychological safety dimension,the scores of personnel who were often in contact with patients(3.59±0.56)were significant higher than those who occasionally/rarely contacted patients(3.37±0.50)(p<0.05).As for education,secondary education college personnel score lower than junior college staff in proactive reporting and they do worse in the factor of team atmosphere than staff with junior college education lever or bachelor degree;while in the factors of humiliation-fear,secondary education college personnel score higher than bachelor-degree staff.Meanwhile,various specialties show significance at factor of proactive report.That is nurses(2.72 ± 1.35)and administrative staff(2.73 ± 1.31)scored higher than doctors(2.23 ± 1.25).Conclusion:1.The level of patient safety culture condition of community hospitals(PSCCH)in Shaoxing is above the medium level of China.The scores of governor superiority,team atmosphere,communication&cooperation,and safety resources are significantly higher than the medium level,which can be ascribed to the regional advantage.Simultaneously,the scores of unsafe behavior,fear of humiliation and psychological security are close to the medium level.However,the proactive report is below the medium level and has the largest variation,which can be regarded as the factor to be improved.2.Community hospital staff with different characteristics have different perceptions level of hospital safety-culture.In general,the clinical medical staff(the community hospital staff who are exposed to patients frequently)have poorer awareness of the hospital safety culture than that of occasional/rear-contact-patients personnel.Besides,nurses and administrative staff score higher than doctors,simultaneously,college and undergraduate medical staff score higher than secondary school personnel.3.The patient safety cultural condition of community hospitals(PSCCH)in Shaoxing area still needs to be further improved.In particular,the clinical staff should be encouraged to report on the occurrence of patients' safety incidents actively,and strive to rectify the condition to improve the safety-culture condition of community hospitals conceptually,institutionally and practically.The ultimate goal is to improve the level of safety-culture condition,create a superb hospital safety-culture and promote the doctor and patients safety.
