ObjectiveThis study is to investigate the influencing factors of the nurses' care failure in Yantai tertiary hospital nurses,and the influencing factors can be analyzed.The study can reduce the incidence of hospital care adverse events,prevent and control.MethodsFrom the three hospitals in Yantai City,through the proportional extraction method to select 332 nurses to carry out the investigation.The survey of the population's sociological information for the survey was conducted through the "general questionnaire";the survey was conducted on the intention of the nurses reporting adverse events through the "nurses'report on the intentions of adverse events";the degree of adverse events Incident reporting questionnaire "for the current care failure is difficult to smooth the status of the report through the" Adverse Event Report Barrier Questionnaire "to investigate;for nurses to understand the attitude of patients through the" nurses on the patient safety attitude questionnaire "to investigate.Results1.General information about the subjects of the studyThe average age of 332 nurses is 31,56 ± 71.62 years old;The length of service range is 10.51 ± 79.61 years;The proportion of women was 96.7 per cent.The proportion of contract/hired nursing staff is 45.5%/54.5%;Junior college education accounts for 59.9%,undergraduate and above accounts for 31.9%;Nursing workers account for 48.8%and nurses account for 27.7%;The deputy director and above account for 1.5%,and the head nurse account for 9.3%.ordinary nurses account for 90.7%;The number of Emergency Services was 29.8%,the proportion of internal medicine was 19.9%,and the lowest was 15.2%in Department of surgery.2.The research object of the report intention situationNursing adverse events report intention questionnaire survey indicated that the median total score of the questionnaire was 13(3?15),P25 was 11,and P75 was 14.Among them:The rate of discussed with colleagues among various types of adverse events is more than 90%,of which the highest is 97.8%for "causing serious injury or death";The relatively lowest rate of intent reported in the statement one is "potential loopholes"(62.7%)and "no harm"(61.2%).3.The research object of the report cognitive situationIn this study,the total cognitive score of the adverse event report was non-normal distribution,in which the total score of the questionnaire was 0 and the maximum value was 8.The median reported cognitive score was 4(0?8),P25 was 3,P75 was 6,and the reported cognitive process was at a moderate level.The two items in the study with relatively high cognitive rates were the related institutional cognitive aspects of the report(93.1%)and the institutional cognitive aspects of the report(92.2%).The lowest level of awareness was whether the report form had been completed(36.0 per cent).4.The report of the research object obstaclesAmong the reporting obstacles:three projects with a high degree of identity which hindered the reporting were:(1)"If you report the analysis results of the report,you can obtain feedback in a timely manner,such as the implementation of relevant and improved countermeasures.It will promote the improvement of its own reporting intentions "(72.4%);(2)"If you can analyze the report and publish the results of the analysis on a regular basis,you will promote the improvement of your own reporting intentions"(72.0%);(3)"If the report is simple and easy to fill out,it will promote the improvement of its own reporting intentions"(69.6%).In addition,the report barrier questionnaire is analyzed from three dimensions of reporting meaning,reporting process,and punitive culture.The total score of each dimension is non-normal distribution data.5.Perception of Patient's Safety Attitudes(1)Active Response Rate of Nurses to the Dimensions of Patient's Safety AttitudesThe percentage of nurses 'positive responses to each item in the patient's safety attitude questionnaire was from high to low.The top four were:(1)"As far as my department is concerned,patients' safety is the first priority,and patients 'safety is attached great importance"(96.3%);(2)"In terms of collaboration between nursing personnel,the departments concerned give encouragement and advocate the development of a sense of cooperation with the team"(93.8%);(3)"When things outside the scope of work interfere with me,I can still strictly abide by the patient's safety first rule"(90%);(4)"In the face of patient safety,each staff member of his or her own department can perform his or her duties with due diligence"(90%).(2)Total Safety Attitudes and DimensionsThe total score of all the safety attitude questionnaires was 3.7714 ± 0.5621,which was more than 3 points,which was slightly higher than the average level.Among them,there are two dimensions with a score of more than 4,namely "team atmosphere" and "unit security awareness." The pressure tolerance dimension score is less than 3 points,which is less than the average level.6 Factors influencing the intent of report adverse events(1)Report Intent of Nursing Adverse Events of Different Demographic Characteristics Research SubjectsThe demographic characteristics used in this study mainly include the characteristics of age,working years,academic qualifications,and technical titles of the subjects studied.Among them,the reported intention score increased with the increase in length of service,age,professional functions,and cultural level,and there were statistical differences between the reporting intentions of different groups in the project,P<0.05;There was a statistical difference between the reported intentions in the different gender groups,P<0.05;There will also be significant differences in the reported intention score between different departments and staff,P<0.05,and in terms of the average rank index,From high to low,they are:"ICU","operating room","internal medicine","surgery","oncology","emergency department","outpatient",and "other departments."(2)Relevance of Report Awareness,Reporting Disorders,Safety Attitudes to Reporting IntentThere is a positive correlation between reporting perceptions,reporting barriers,and security attitudes and reporting intentions;There are significant differences in reporting intentions between different reporting perceptions,reporting barriers,and safety attitudes(P<0.05);Among them,there are significant differences between the four dimensions of "job satisfaction","stress tolerance","unit safety identity" and "management cognition" in the safety attitude(P<0.05).However,there is no significant difference between the reporting intention and the team atmosphere in this dimension(P>0.05).Conclusion1.In the absence of adverse events that lead to injury and potential loopholes,caregivers have shown a lower initiative to report their intention to report an increase in the severity of adverse events.2.Nursing staff in reporting adverse events with their age,length of service,cultural level,professional titles,working departments and the impact of the types of medical departments;nurses report the intention of work adverse events with their length of service,cultural level,Safety awareness and reporting barriers.
