Research backgroundMedical adverse events is one of the major problems of public health worldwide.The ubiquitous occurrence of adverse events has been unveiled with clarity and determination,and have brought a lot damage to patients and medical staff.Through being reported and analyzed, actions can be taken according to the results of the analysis,to prevent the recurrence of adverse events,which is most recommended by many medical institutions.However,the reporting rate is very low.Research purposeBy using questionnaire survey to nurses working in general hospital in Tai’An, we would evaluate the reliability and validity of the structure of adverse events risk perception among clinical nurses.The aim of this study is to describe the present condition and influencing factors of adverse events risk perception and adverse events report habits among clinical nurses, as well as relationship of them,in order to provide theoretical basis on improving nurse reporting adverse event.Materials and methodsThe draft question naire was formed by semi-structured interview and open questionnaire survey,and then was revised through Delphi Method and apreliminary test.By the whole group of convenience sampling method,we selected 1080 nurses in 10 hospitals in Tai’an. Participants completed general information questionnaire,Clinical nurses adverse events risk perception structure Scales,Adverse events report habits Scales.The data were input by excel 2007 and analyzed by SPSS 19.0 and AMOS 17.0.Main Results1. A scale of three factors and 20 items was identified byusing item analysis and exploratory factor analysis and the three factors could explain 60.374% of the variance.The confirmatory factor analysis revealed a good fit of the model to the data based on various fit indices (X2/df=2.427;GFI= 0.936;AGFI= 0.917;RMSEA =0.049;NFI=0.915;CFI=0.9 48;IFI=0.948).The Cronbach’s acoefficient of the adverse events risk perception scale was 0.939 and the Cronbach’s a coefficients of each factor were all above 0.7.The correlations of the scores between each item and the total scale were ranged from 0.46 to 0.77 (P<0.01).Exploratory and confirmatory factor analysis revealed that there were three dimensions of adverse events risk perception: patient care risk,System process risk, and nurse operation risk.2. The score of clinical nurses adverse events risk perception was (16.48±3.79). The score of each dimension in descending order were:nurse operation risk, System process risk, and patient care risk. Work experience,departments and hospital level were significant influencing factors of clinical nurses adverse events risk perception.The score of clinical nurses adverse events report habits was (36.80±19.71). The score of each dimension in descending order were:nurse operation adverse events, System process adverse events, and patient care adverse events. Hospital level were significant influencing factors of clinical nurses adverse events report habits. Pressure sores are most frequently reported, and the nurse manpower allocation is not reasonable cause patient injury report the lowest frequency.Adverse events report habits was significant positive correlated with adverse events risk perception.Patient care risk perception was indicated as predictive factors of adverse events report habits among clinical nurses.Conclusions and suggests1. The multi-dimensional structure of adverse events risk perception indicates the characteristics of these nursing service. The adverse events risk perception scale is reliable and valid, and can be used to measure adverse events risk perception among cinical nurses.2. The level of clinical nurses adverse events risk perception need to be improved in general hospital in Tai’an.3. Adverse events reporting habit of clinical nurses is poorer in general hospital in Tai’an.4. The level of clinical nurses adverse events report habits can be influenced by patient care risk perception.5. The level of clinical nurses reporting adverse event need to be improved. Nurse managers should pay attention to improve nurse risk awareness and adverse events report habits。we can improve nurse reporting adverse event by increasing nurse risk awareness to better promote of the safety of patients and reduce occurrence of adverse event.6. Suggests:The learning-oriented adverse events management team was set up composed by professional team. The team using event reports data to improve safety culture and level of clinical nurses adverse events risk perception,form hospital reporting and learning systems community of practice.The team can improve management efficiency and ensure patient safety.
