Objective:Based on the analysis of railway workers' health status,the basic situation and distribution rule of chronic diseases were found.To analyze the possible factors that cause the high incidence of certain chronic diseases,and to lay emphasis on the prevention and treatment of chronic diseases in railway workers.To explore preventive measures and intervention measures,and to provide evidence for drawing up preventive measures for chronic diseases.Methods:Select the physical examination data and lifestyle questionnaire data of railway workers in a railway station in Qiqihar in 2017.The inclusion criteria were(1)the lifestyle questionnaire indicated that there was a chronic disease history;(2)the physical examination data were in accordance with the relevant contents of the national standard for the prevention and control of chronic diseases.The examination included age,sex,height,weight,blood pressure,blood routine(leukocyte,red blood cell,hemoglobin,platelet),liver function(serum alanine transferase ALT,aspartate transferase AST,gamma glutamyl transferase GGT),renal function(blood creatinine Cr,blood urea nitrogen BUN,serum uric acid UA).Fasting blood sugar,blood lipid(total cholesterol TC,triglyceride TG),urine routine(urine occult blood,urine protein,urine sugar,urinic cells),chest X-ray film,color B ultrasound(liver,bile,spleen,pancreas + double kidney),electrocardiogram.EXCEL was used to build database,and SPSS24.0 statistical software was used for statistical analysis of data,and then single factor analysis and multivariate analysis were performed.Results:1.Types of Chronic Diseases of All Employees Among the 2006 physical examination workers,types of chronic diseases:498 cases were hyperlipidemia,accounting for 24.86%;200 cases were coronary heart disease,accounting for 9.98%;189 cases were chronic bronchitis,accounting for 9.42%;500 cases were hypertension,accounting for 24.93%;136 cases were cerebral infarction,accounting for 6.78%;37 cases were emphysema,accounting for 1.84%;152 cases were diabetes,accounting for 7.58%;23 cases were cerebral hemorrhage,accounting for 1.15%.2.The single-factor analysis of different chronic diseases Comparison of hyperlipidemia among workers with,age,blood pressure,BMI,white blood cell,hemoglobin,urinary protein,urinary sugar,ALT,AST,fasting blood glucose,total cholesterol,triglyceride,high density lipoprotein,low density lipoprotein,electrocardiogram,liver,biliary tract,left kidney,right kidney,family history,malignant tumor Tumor,diet,smoking,drinking,sleep,P < 0.05;The prevalence of coronary heart disease among workers of,age,blood pressure,BMI,hemoglobin,urinary leukocyte,urinary sugar,fasting blood glucose,triglyceride,low density lipoprotein,electrocardiogram,liver,biliary tract,right kidney,family history,diet,smoking,drinking and sleeping were compared,P < 0.05;The prevalence of chronic bronchitis among workers with different urinary protein,uric acid,abnormal chest Xray,family history,diet and smoking were compared,P < 0.05;Different blood pressure,BMI,hemoglobin,urinary protein,urinary sugar,AST,fasting blood glucose,triglyceride,electrocardiographic abnormalities,liver abnormalities,biliary tract abnormalities,left kidney abnormalities,right kidney abnormalities,chest X-ray abnormalities,family history,diet,smoking,drinking workers with hypertension were compared,P < 0.05;The incidence of cerebral infarction in workers with,age,blood pressure,BMI,hemoglobin,urinary occult blood,urinary sugar,ALT,AST,urea nitrogen,fasting blood glucose,ECG abnormalities,liver abnormalities,biliary tract abnormalities,pancreatic abnormalities,right kidney abnormalities,chest X-ray abnormalities,family history,diet,smoking and drinking were compared,P < 0.05;The incidence of emphysema in workers with,AST,fasting blood glucose,pancreatic abnormality,left kidney abnormality,chest X-ray abnormality and diet were compared,P < 0.05;The prevalence of diabetes mellitus in workers with,age,blood pressure,BMI,hemoglobin,urinary protein,urinary sugar,AST,fasting blood glucose,triglyceride,electrocardiogram,liver,biliary tract,family history and alcohol were compared,P < 0.05;The cerebral hemorrhage of workers with,age,fasting blood glucose,abnormal electrocardiogram,abnormal biliary tract,right kidney,family history,diet,smoking were compared,P < 0.05,the difference were statistically significant.3.The multivariate Analysis of Influencing Different Chronic Diseases Logistic regression showed that: age,blood pressure,BMI,total cholesterol,highdensity lipoprotein,liver abnormalities,family history,malignant tumors,drinking,sleep were the factors affecting the incidence of hyperlipidemia in physical examination workers;Age,abnormal electrocardiogram,family history and sleep condition were the factors that affected the incidence of coronary heart disease in physical examination workers;Uric acid,abnormal chest radiography and family history were the factors that affect the incidence of chronic bronchitis in physical examination workers;Gender,age,BMI,urinary protein,fasting blood sugar,electrocardiogram,liver,kidney(left),family history and alcohol consumption were the factors affecting the incidence of hypertension in physical examination workers;Age,urine occult blood,pancreas,right kidney and family history were the factors affecting the incidence of cerebral infarction in physical examination workers;Fasting blood glucose,abnormal pancreas,abnormal chest X-ray and dietary status were the factors affecting the incidence of emphysema in physical examination workers;Age,urine sugar and family history were the factors that affect the incidence of diabetes in physical examination workers;Age and family history were the factors that affect the incidence of cerebral hemorrhage in physical examination workers.4.Main Influencing Factors of Eight Chronic Diseases The family history is the common influencing factor of seven diseases such as hyperlipidemia,coronary heart disease,chronic bronchitis,hypertension,cerebral infarction,diabetes mellitus and cerebral hemorrhage;age was the common influencing factor of seven diseases such as hyperlipidemia,coronary heart disease,hypertension,cerebral infarction,diabetes mellitus and cerebral hemorrhage;electrocardiogram abnormality was the common influencing factor of hyperlipidemia,coronary heart disease,hyperemia,etc.Common influencing factors of three diseases of hypertension: abnormal liver was the common influencing factor of two diseases of hyperlipidemia and hypertension;BMI and sleep were the common influencing factors of two diseases of hyperlipidemia and coronary heart disease;alcohol consumption is the common influencing factor of two diseases of hyperlipidemia and hypertension;abnormal chest X-ray is the common influencing factor of two diseases of chronic bronchitis and emphysema.Common influencing factors: urine sugar was the common influencing factor of hypertension and diabetes mellitus.5.Analysis of the correlation of chronic diseases Correlation analysis of chronic diseases showed that hyperlipidemia was associated with coronary heart disease,chronic bronchitis,hypertension,cerebral infarction,emphysema,diabetes,and cerebral hemorrhage(P<0.05);Coronary heart disease associated with hyperlipidemia,chronic bronchitis,hypertension,cerebral infarction,emphysema and diabetes(P<0.05);Chronic bronchitis was associated with hyperlipidemia,coronary heart disease,hypertension,cerebral infarction,emphysema and diabetes(P<0.05);Hypertension was associated with hyperlipidemia,coronary heart disease,chronic bronchitis,cerebral infarction,emphysema,diabetes,and cerebral hemorrhage(P<0.05);Cerebral infarction and hyperlipidemia,coronary heart disease,chronic bronchitis,hypertension,emphysema,diabetes and cerebral hemorrhage(P<0.05);Emphysema associated with hyperlipidemia,coronary heart disease,chronic bronchitis,hypertension,cerebral infarction and diabetes(P<0.05);Diabetes and hyperlipidemia,coronary heart disease,chronic bronchi Inflammation,hypertension,cerebral infarction and emphysema were associated(P<0.05);Cerebral hemorrhage was associated with hyperlipidemia,hypertension and cerebral infarction(P<0.05).Conclusion:1.In 2017,the overall health of railway staff at a station in Qi area was generally.The prevalence rates of hyperlipidemia and hypertension were both over 20%.The prevalence rates of coronary heart disease and chronic bronchitis were close to 10%.Cardiovascular and cerebrovascular diseases and pulmonary diseases were common chronic diseases;2.age,blood pressure,BMI,hemoglobin,urinary protein,urinary sugar,AST,fasting blood glucose,triglyceride,electrocardiogram abnormalities,liver abnormalities,biliary tract abnormalities,left kidney abnormalities,right kidney abnormalities,chest X-ray abnormalities,family history,diet,smoking,drinking and other factors have an impact on different chronic diseases;3.Hyperlipidemia,coronary heart disease,chronic bronchitis,hypertension,cerebral infarction,emphysema,diabetes,and cerebral hemorrhage all showed different degrees of correlation.Preventive measures for chronic diseases:According to the physical examination status of railway employees,assess and analyze their own health status,and the risk factors of employees should be assessed well at the same time.On the basis of qualitative assessment,analyze the main risk factors of individuals,so that better According to the nature and characteristics of the individual,the individual's disease type is analyzed,and the means of classification management are adopted to take effective preventive measures.
