Objective: We studied the epidemiological features of polypoid lesions of the gallbladder(PLG)and gallstone among the railway workers to figure out the risk factors and the causes which affect the development of PLG and gallstone.The main purpose is to give submental evidences may help preventing and controlling PLG and gallstone to protect the health of railway workers.Methods:(1)The study including a total of 1069 subjects who underwent PLG found in the year of 2016 among railway workers of A Railway Group Company in Hunan and Guangdong during health screening examinations,comp-aring with a total of 14336 subjects without PLG who also had taken physical examinations and blood test in the same year,and a total of 925 subjects with gallstone comparing with a total of 12779 subjects without gallstone.(2)The comparison of differences between railway workers with PLG and gallstone and those who without PLG and gallstone has been analyzed by cross-sectional study.(3)We comparing the demographic factor,clinical indicators and fatty liver status of the case group and the control group.After that,the univariate analysis and multivariate regression analysis were conducted.Results:(1)Overall prevalence of PLG is 6.94%.The PLG prevalence is7.76% and 3.76% in male and female,respectively.Those who are 50 years and elder have the highest prevalence which is 8.38%.The highest prevalence ofPLG among different areas is 14.29% in Chenzhou.The highest rate among different positions is 7.65% in locomotive depots.Overall prevalence of gallston-e is 6.74%,5.77% male and 5.88% female underwent gallstone.The highest rate of prevalence is 8.23% among railway workers above 50 years old.The highest prevalence of gallstone among different areas is 8.46% in Huaihua.Also the trainmasters are the highest prevalence position which is 7.04%;(2)The univariat-e analysis between the PLG group and control group,age,sex,BMI,blood gluco-se,UA,ALT,TBil,total cholesterol and HDL-C are statistically significant differ-ent;The univariate analysis between the gallstone group and control group,age,systolic blood pressure,diastolic blood pressure,BMI,blood glucoseo,urine gluco-se,urine protein,TG,HDL-C,prevalence of fatty liver are statistically significant different(all P values<0.05);(3)Multivariate logistic regression analysis on PLG indicate that old age,male,high BMI,high blood glucose,high total cholesterol,lo-w HDL-C are independent risk factors.The prevalence increase to 1.26 times per 10 years elder(95%CI:1.126~1.410);The risk of gallbladder polyps in female workers is 0.579 times higher than that in male workers(95%CI:0.472~0.710);Those who are fatty have a 1.473 times of risk suffering PLG(95%CI:1.224~1.774);Those who have higher blood glucose have a 1.246 times of risk suffering PLG(95%CI:1.050~1.477);Those who have higher Total cholesterol have a 1.224 times of risk suffering PLG(95%CI:1.049~1.428);Those who have lower HDL-C have a 1.494 times of risk suffering PLG(95%CI:1.230~1.815).Multivariate logistic regression analysis on gallstone indicate that old age,highBMI,fatty liver,high blood glucose,low HDL-C are independent risk factors.The prevalence increase to 1.347 times per 10 years elder(95%CI:1.186~1.531);Thos-e who are overweight have a 1.383 times of risk suffering gallstone(95%CI:1.097~1.744);Those who are fatty have a 1.424 times of risk suffering gallstone(95%CI:1.213~1.670);Those who have fatty liver have a 1.179 times of risk suffering gallstone(95%CI:1.009~1.377);Those who have higher blood glucose have a 1.508 times of risk suffering gallstone(95%CI:1.261~1.803);Those who have lower HDL-C have a 1.261 times of risk suffering gallstone(95%CI:1.007~1.579).Conclusion:(1)The risk factors of PLG are old age,male,fatty,high blood glucose,high total cholesterol and low HDL-C;(2)The risk factors of gallstone are old age,overweight,fatty liver,high blood glucose and low HDL-C;(3)The common risk factors both in PLG and gallstone are old age,fatty,high blood glucose and low HDL-C.
