| ObjectiveThis study collected the case date of gout with“Six symptoms”patients which include gender,age,course of disease,seasonal etiology,syndrome of TCM and related laboratory index.To study and analyze the clinical features of traditional Chinese Medicine,which can provide the basis for diagnosis and treatment of gout with "six Symptoms" and prevention of gout and its complications.MethodsThis study use retrospective study method,which select case data of gout with "six symptoms" from the case data of hospitalized patients who was diagnosed with gout with "six Symptoms from April 2016 to April 2017,and inputted and verified the data to be statistic analyzed by SPSS 22.0.Results1.Basic data:Male patients accounted for 93.5%by 101 cases,female predilection patients accounted for 6.5%by 7 cases,the mean age of onset is 54.0±12.4 years and predilection age is 50-60 years.In twenty-four solar terms,the highest incidence is Guyu,Qingming and Daxue,next was Lichun and Xiaohan.Gout in acute attack mostly induced by high-purine diet,accounted for 76%by 54 cases,followed by alcohol,accounted for 45%by 32 cases,the others are trauma,climate change,catch cold.Gout with "six symptoms"with family medical history accounted for 28.7%,the highest proportion was hypertension,diabetes,cerebrovascular disease.The main affected joints was ankle,accounting for 70.40%.The highest tophi attacked was first plantar joint toes,the followed part were elbow joint and metacarpophalangeal joint.2.Gout with "Six Symptoms" and associated other comorbidities:Gout with"two symptom" accounting for 47.2%by 51 cases in which the highest proportion is gout with hypertension;gout with "three symptoms" accounting for 31.5%by 34 cases in which gout with hypertension and obesity are the most;gout with "four symptoms" accounting for 16.7%by 18 cases in which gout with hypertension,obesity,hyperlipidemia are the most;gout with "five symptoms" accounting for 3.7%by 4 cases;gout with "six symptom" accounting for 0.9%by 1 case.The highest percentage of gout with "six symptom" combined other diseases is fatty liver by 53.7%,the followed diseases are osteoarthritis,kidney stones,osteoporosis,coronary heart disease.3.Gout with "Six Syndrome" of damp-heat accumulation type and Non damp-heat accumulation type:for two group of patients with tophi have no significant difference(P=0.709>0.05)which have comparability.Age composition have significant difference(Mann-Whitney U=1077.50,p=0.02<0.05)·Gender composition have significant difference(P=0.020<0.05)which have comparability.Course composition have no significant difference(Mann-Whitney U=1349.50,p=0.589>0.05).Gout with“Six Syndrome”composition have no significant difference(Mann-Whitney U=1419.00,p=0.923>0.05).The two group with laboratory index of uric acid,serum creatinine,erythrocyte sedimentation rate(ESR)and C-Reactive Protein(CRP)have no significant difference by statistical analyzing.ConclusionThe study found that gout patients with "Six symptoms" occupy a higher proportion among the gout patients,predilection age is 50-60 years,the duration of 5-10 years are most common,which mostly induced by high-purine diet,alcohol consumption,trauma,and patients are easily attacked in winter and spring season alternation.Gout patients with“Six symptoms”are mostly syndromes of damp-heat accumulation type,Chinese medicine treatment should be clear heat and phlegm stasis,while care spleen and kidney,gout patients should prevent hypertension,obesity,hyperlipidemia in advance.Gout with“six symptoms " combine the other diseases are usually fatty liver and osteoarthritis. |
