The Preliminary Study Of Traditional Chinese Medicine Syndrome Distribution Of Chronic Gout

ObjectiveThe objective of our study is to preliminary discussion the syndrome distribution characteristics and laws on chronic gout. Establishing its main syndrome and elements,in order to provide a reference for gout clinical treatment.MethodsIn this study, we use epidemiological cross-sectional survey research method,design the clinical questionnaire,then investigate and acquire the relevant data.Then we adopt EpiData3.1software for data management,use SPSS16.0statistical analysis software to carry on the frequency analysis,analysis of variance, factor analysis, Logistic regression,Discriminant analysis.Based on these methods,we analysis the syndrome distribution characteristics and laws of chronic gout.Results1Clinical characters:The incidence rate of gout in patients is men more than women, the ratio is32.3:1.Onset age concentrates in30-60years old. It can come on all the year.However, spring and season alternative will affect the condition. High purine diet and alcohol is the main cause of gouty arthritis attack.The course of this disease focuses on1-5years.Obesity, Abnormal Metabolism of Blood fat, High Blood Pressure and Fatty Liver Disease are the maincomplicatig diseases.2Clinical symptoms:The first plantar toe joint is the main pain part of chronicgout patients.The initial symptom is joints’s inflamed hot pain. The joints stabbing pain is always day light night heavy.The color of the pain parts is dark red or purple. About91(75.8%) patients have tophus, which occurred in plantar toe joint is about58(49%) cases.The most significantsystemic symptom is joints heavy.The tongue is:old, dark purple and yellow greasy coating.The pulse condition is heavy slipping. Sublingual collaterals is more circuity.3Syndrome distribution:In this survey,chronic gout syndrome distribution is as follows:phlegm-stagnancy-type is the mostcommon syndrome,then in proper order separately is spleen deficient-type,liver and kidney Yin deficiency-type, damp-heatretention-type.However,the clinical syndrome is complex.The most common combination is:phlegm-stagnancy-type with spleen deficient-type or phlegm-stagnancy-type with liver and kidney Yin deficiency-type,accounted for20.0%,20.0%respect-ively.4Chronic gout syndrome differentiation factors:Using frequency analysis method to analysis the symptoms, finding that the frequency is greater than or equal to50%of the symptoms are:tophus joints stabbing pain,the first plantar toe joint pain,dark purple,yellow tongue,day light night heavy,the parts color purple darkjoints deformity,body and joints heavy,sublingual veins purple.Also acquiring that dark purple,excessive phlegm, tophus, joints stabbing pain, headache is the main dialectical elements of phlegm-stagancy-type.Accordingly,Fisher’s discriminant function is established,of which the judgment accuracy model is78.3%, the sensitivity is83.3%.5Chronic gout treatment principle:Under the guidance of TCM’s basic principle syndrome differentiation and treatment,aim at the main syndrome,follow the main principle which treat both manifestation and root cause,adopt phlegm drain turbidity,activate blood and eleminate stasis.Otherwise,according to the virtual condition,respectively given spleen wet permeabiliyor stren-gthen the liver and kidney.Conclusion1Phlegm and blood stasis is the key point of the pathogenesis and throughout the course of the Chronic gout.2Chronic gout syndrome distribution:phlegm-stagnancy-type is the most common syndrome, then in proper order separately is spleen deficient-type,liver and kidney Yin deficiency-type,damp-heat retention-type.However,the clinical syndrome is relatively complex,withcomplex syndrome is seen.One of the most common combination is: phlegm-stagnancy-type with spleen deficient type or phlegm-stagnancy-type with liver and kidney Yin deficiency-type.3Chronic gout main syndrome is phlegm stagnancy type, the main the main dialectical elements of which are:dark purple, excessive phlegm, tophusjoints stabbing pain,headache.And the main symptoms are:tophus,stabbing pain,the firstplantar toe joint pain,dark purple,yellow tongue,day light night heavy,jointsdeformity,body and joints heavy,sublingual veinspurple.4Deficiency in origin and excess in superficiality is the pathogenesis character of chronic gout,so at the time of treatment should be both.To drain turbidity of blood,activate meridians to stop pain,and also should be given spleen wet permeability or strengthen the liver and kidney.5Rule of syndrome standardization study is the inevitable trend in the modernization of traditional Chinese medicine,the syndrome of correlation analysishas become the key points and difficulties of related research.Standardization and normalization of syndrome study is the key of TCM to to warding standardization,objectification and quantification.This study which offers the disease syndrome research ideas and methods by using clinical epidemiological investigation method combined with statistics to justify syndrome studied isproved to be feasible.
