Observation On The Efficacy Of Huangdi Nei Zhen Combined With New Bobath Technique In Hemiplegic Lower Extremity Spasm After Stroke

Objective: To observe the clinical efficacy of Huangdi Nei Zhen in the treatment of hemiplegic lower extremity spasm after stroke,and to compare its clinical efficacy with that of traditional acupuncture.Methods: In this study,90 patients who met the inclusion criteria were randomly divided into group A(30 cases of Huangdi Nei Zhen combined with New Bobath technology),group B(30 cases of traditional acupuncture combined with New Bobath technology)and group C(30 cases of New Bobath technology)by drawing lots according to the order of admission.The basic treatment of neurology in each group was in accordance with the plan of "guidelines for the prevention and treatment of cerebrovascular diseases in China"(2005 edition),to regulate blood pressure,blood glucose,blood lipids,anti-platelet aggregation and neurotrophic therapy,as well as the treatment of symptomatic treatment and prevention and treatment of complications.Acupoint selection and positioning of traditional body acupuncture therapy:refer to the ninth edition of the standardized textbook of traditional Chinese medicine on education in colleges and universities,and select the lower limb huanyue,bi guan,yanglingquan,zusanli,jiexi and kunlun on the affected side.Huangdi Nei Zhen treatment in accordance with the four general principles of internal acupuncture point selection according to the 6321 diagnosis and treatment ideas,six: meridians dialectical,six meridians dialectical,clear spasm site;Three: three ability dialectical--three jiao dialectical,clear spasmodic three jiao attribution part;Two: Yin and Yang are changed.According to the principle of treating the lower and upper diseases and treating the left and right diseases,select the upper focal part of the upper limb on the opposite side of the affected limb(left to right,right to left),select yanggu plus branch of the sun meridian,shenmen plus tongli of shaoyin meridian,yangchi plus external pass of shaoyang meridian,and daling plus internal pass of jueyin meridian.Treatment course: once a day for 5days/week for 4 weeks.Evaluation of curative effect: before and after treatment,patients were assessed by the internationally used improved Ashworth scale,lower limb clinical spasm index(CSI)and improved Barthel index to evaluate the effect after treatment,and SPSS 21.0 statistical software was used for data analysis.Results: 1.MAS scale analysis: comparison of the three groups before and after treatment,P < 0.05,the difference was statistically significant,indicating that the three groups were effective in reducing lower limb spasm;Pairwise comparison between the three groups after treatment showed that P <0.05,indicating that the combined treatment was more effective than the simple treatment,while the efficacy of Huangdi Nei Zhen combined with the New Bobath technology was even better.2.CSI scale analysis: the total scores of the three groups before and after treatment were compared(P < 0.05),and the difference was statistically significant,indicating that the three groups could improve the degree of lower limb spasm.After treatment,the three groups were compared in pairs,group A and group B(P < 0.05),group A and group C(P < 0.05),and group B and group C(P < 0.05).This indicated that the combined treatment was more effective than the single treatment in the three groups,while the efficacy of Huangdi Nei Zhen combined with the New Bobath technology was better.3.ADL scale score: all three groups were compared before and after treatment(P < 0.05),indicating that all three groups could improve their daily activities.Pairwise comparison of the three groups after treatment(P < 0.05)indicated that the effect of combined treatment was better than that of single treatment,in which the effect of Huangdi Nei Zhen combined with New Bobath technology was even better.Conclusion: Group A,B and C all showed good efficacy in the treatment of stroke hemiplegic lower limb spasm.The combined treatment effect of group A and B was better than that of group C with A single treatment scheme,and the effect of group A was even better.
