BackgroundCoronary atherosclerotic heart disease refers to the heart disease caused by coronary artery atherosclerotic lesions which lead to vascular stenosis or obstruction,myocardial ischemia,hypoxia or necrosis.It is often called "coronary heart disease".In recent years,the incidence of cardiovascular diseases has increased significantly.Coronary artery atherosclerosis has become the most common cause of death in the world.In recent years,many studies have proved that atherosclerosis is mediated by chronic inflammation,among which dyslipidemia,hypertension and diabetes play an important role.Complement is the main system in innate immune of the host,and its involvement in inflammation also plays an important role in the pathogenesis of coronary atherosclerotic heart disease.Complement 1q is a subcomponent of Complement 1,which plays a key role in triggering the activation of classical pathways by identifying immune complexes.However,little is known about its role in chronic and low-grade systemic inflammation,which plays an important role in metabolic diseases such as atherosclerosis.More and more basic studies have found that C1q can directly interact with macrophages to inhibit plaque progression.At present,coronary angiography is the golden standard for the diagnosis of coronary atherosclerotic heart disease,but it is an invasive operation with certain risks and relatively high costs.There are still hospitals that have not carried out,so a simple and feasible index is needed for preliminary screening.ObjectiveTo analyze the relationship between coronary heart disease(CHD)with type 2 diabetes mellitus(T2DM)and the level of serum Clq and other biochemical indicators.To explore the correlation between serum C1q level and coronary heart disease with T2DM.MethodsA total of 117 patients with suspected coronary heart disease who underwent coronary angiography were selected.Thirty-eight patients were divided into coronary heart disease combined with T2DM group,including 25 males and 13 females,with an average age of(61 + 12)years old;36 patients were divided into coronary heart disease without T2DM group(simple coronary heart disease group),including 21 males and 15 females,with an average age of(64 + 13)years old;21 patients were divided into without coronary heart disease combined with T2DM group(simple T2DM group),including 12 males and 9 females,with an average age of(63 + 8)years old;22 patients were divided into the control group(no coronary heart disease without T2DM group),including 13 males and 9 females,with an average age of(63 + 9)years old.There was no significant difference in sex and age between groups(P > 0.05).After admission,peripheral venous blood was collected and strictly controlled before Percutaneous Coronary Intervention.Serum C1q,fasting blood glucose,triglyceride,total cholesterol,low density lipoprotein cholesterol and high density lipoprotein cholesterol were measured.SPSS 19.0 software was used for statistical analysis.Results1.There were no significant differences in age,height,weight,HDL-C(high density lipoprotein cholesterol),total cholesterol,LDL-C(low density lipoprotein cholesterol)and serum creatinine between groups(P > 0.05).There were significant differences in C1q,triglyceride and fasting blood glucose among the groups(P < 0.05).2.LDS-t test showed that the level of serum C1q in coronary heart disease with T2DM group was significantly higher than the other three groups,and the difference was significant(P < 0.05);There was no significant difference in the level of serum C1q among the other three groups(P > 0.05).3.Spearman correlation analysis was made between serum C1q level and coronary heart disease combined with T2DM group.The Spearman correlation coefficient Sig(P value)= 0.02,which means a significant correlation between elevated serum C1q level and coronary heart disease combined with T2DM.4.Using ROC curve to compare the level of serum C1q with coronary angiography,the diagnostic value of serum C1q level for coronary heart disease was detected.The area under the ROC curve was 0.566,P=0.235.There was no statistical significance,indicating that C1q had no significance in the diagnosis of coronary heart disease.Use the same method to detect diagnostic value of serum C1q level in coronary heart disease combined with T2DM.The area under the ROC curve was 0.699,P=0.002.The results showed that serum C1q level had significance in the diagnosis of coronary heart combined with T2DM.When the serum C1q level was 234.5 mg/L,the maximum value of the Youden’s index was 0.416.At the same time,the sensitivity and specificity of the serum C1q level for the diagnosis of coronary heart disease combined with T2DM were both higher.ConclusionsThe level of serum C1q increased in patients who have coronary heart disease,especially in patients who have coronary heart disease with T2DM.The elevated level of serum C1q was correlated with coronary heart disease and type 2 diabetes mellitus.Except for inflammation,infection and cancer which affect the level of serum C1q,the elevated level of serum C1q in type 2 diabetes suggests that coronary heart disease may be associated.
