Background:With the continuous improvement of image inspection technology,especially,the development of MRI technology,it has become a reality that people can study the brain structure and function in vivo state.Combined with the rapid development of computer technology,doctors can accurately localize brain lesions,perform minimally invasive treatment,such as micro-neurosurgery,stereotactic neurosurgery,which makes the previous "brain lobe range Surgery" transit to the "lesion range surgery" level.The Planum Temporale(PT)of the brain is an approximate triangular cortical region located at the base of the lateral fissure,the posterior edge of the temporal gyrus.The previous definition of the boundary of the temporal plane has been disputed.Until 1975,Juhn A.Wada,A Canadian medical doctor,who defined the boundary of The Planum Temporale,for the first time,by the study of 100 adults and 100 infant cadavers: it is located at the bottom of the lateral fissure,whose anterior boundary is the temporal transverse gyrus(Heschl's gyrus,HG),the posterior boundary is the transition point of the same lateral lateral fissure,and the lateral boundary is an approximate triangular cortical region of the lateral margin of the temporal transverse,which is an important association area of auditory language cognition,and also one of the representative structures of the cerebral dominance.In the the 1970 s,Yasarisl proposed the use of natural anatomical space in the skull and the brain to reach the lesion site and treat the lesions,which can reduce surgical injuries and protect the function of the brain and nerves.Meanwhile external lateral fissure,the most prominent lateral brain of the normal natural fracture of the brain,is the surgery preferred approach to deep brain and adjacent structure,so it is very important to recognize and protect the surrounding structure.In this experiment,30 normal adults(15 males,15 females)brains were underwent MRI scans with AC-PC as the baseline,to obtain the transverse,coronal and sagittal images.We studied the position and morphology of the Planum Temporale of the living brain,established a three-dimensional coordinate datasets,and constructed a three-dimensional visualization model of the Planum Temporale based on the whole brain.It provides anatomical reference for stereotactic surgery,interventional radiology and minimally invasive neurosurgery involving the Planum Temporale and its surroundings,and new teaching approach for clinical anatomy teaching.Objective:1.To study the morphological characteristics of the planum temporale of the brain on the transverse,coronal and sagittal images by MR images with AC-PC line as the scanning baseline.2.To establish the three-dimensional location dataset of the medial edge of the temporal plane in Cartesian three-dimensional coordinate system,and analyze morphological features and lateral differences.3.To construct a three-dimensional visual model of the brain's temporal plane with a healthy adult's MR Images,to explore the anatomical features of the planum temporale and the application of stereotactic surgery.Methods:1.30 healthy normal volunteers' brains were scanned by MR with the AC-PC line as the scan baseline.The scanning data were imported into e Film 2.1 Workstation in Dicom3.0 format on microcomputers,and the shape and position of the planum temporale on the coronal plane in the continuous transverse plane were observed by using the "3D-cursor" technique.2.The above 30 cases of healthy adult brain coronal MRI data were transformed into Photoshop software and then panned the coordinate origin translation,so that we can make the coordinate system of the software coincide with the system based on AC-PC line.The coordinates of the planum temporale at each image were measured,and the X and Z coordinates of each sampling point were recorded.Y values can be got through product of the thickness and number of the layers.Finally,the coordinate values of all sampling points were converted to the coordinates of the brain space coordinate system.3.A normal adult head MRI coronal T1 WI data was imported directly into the3d-doctor software in Dicom3.0 format,using the manual segmentation method to establish three-dimensional model and mark related structure with different color.Results:1.The planum temporale can be accurately identified in the continuous layer ofthin-layer MR images,and the morphological and positional laws of the temporal plane were obtained:(1)Imaging anatomy of the planum temporale of the brain in axial sections: due to the anatomical location of the planum temporale,firstly,we should identify the temporal transverse gyrus(helchl ' gyrus)on the axial section,and the temporal transverse gyrus can be found in two forms: “Y”shape and“single finger” shape;The gray matter cortex,behind a small amount of cerebrospinal fluid,was the planum temporale.(2)Imaging anatomy of the planum temporale of the brain in coronal sections: the planum temporale was easier to identify on the coronal section than in the axial section,and the temporal transverse gyrus was still recognized first: the first cerebral gyrus at the horizontal part of the lateral fissure was the temporal gyrus,and the dorsal lobe of the lateral lobe of the temporal gyrus was the temporal transverse gyrus,and the coronal section of the temporal transverse gyrus was " ?-shaped".The planum temporale extended beyond the lateral temporal gyrus of the temporal lobe to the cortex on the lateral side of the brain.(3)Imaging anatomy of the planum temporale of the brain in coronal sections:The planum temporale was also easier to identify on the sagittal section than in the axial section,and the planum temporale was located in the cortex of the lateral temporal gyrus at the rear of the temporal transverse gyrus,and the starting point was the end of the posterior margin of the temporal transverse gyrus to the starting point of the anterior lateral fissure.2.Successful construction of normal Chinese adult male and female planum temporale medial margin in the brain of three-dimensional spatial stereo location data set.3.Obtained the space fitting equation of the medial margin of planum temporale.4.The three-dimensional visual model of the planum temporale was reconstructed in the whole brain.the two-sided morphology of the planum temporale were basically similar,and the bottom was the posterior part of the upper part of the superior temporal lobe,which pointed to the cortical structure of the triangle in the deep part of the brain,with a slightly sunken and uneven surface.PT was an acute angle to the horizontal plane.Conclusion:1.For the recognition of planum temporale,using "3d-cursor" technology combined with "continuous tracing" technology,we must first indirectly identify peripheral temporal transverse gyrus and lateral fissure with the help of peripheral structure.The planum temporale appeared as an irregular narrow strip type cortex in the transverse section from the anterior to the outside,and gradually disappears from the upper to the lower side.The planum temporale on the sagittal and coronal sections was higher-posterior and lower-anterior narrow-striped cortex,and from the outward internal accompanying lateral temporal transverse back to disappear gradually.It was easier to distinguish planum temporale on coronal and sagittal section than axial section.2.There was obvious asymmetry in the medial margin of the planum temporale.The medial edge of planum temporale gradually got close to the center of the brain from the frontal to the occipital side,and the tendency to focus on the occipital side;there was no obvious concentration trend in coronal and sagittal plane;the correlation between X,Y and Z is more closely but the regression relationship between the three-dimensional coordinates was not significant.3.Using MRI data to establish three-dimensional planum temporale model,it was important to observe the three-dimensional planum temporale and its adjacent relationship with some important structures from different views,and to carry out three-dimensional anatomical measurement and stereotactic radiosurgery in the model.
