Investigation Of Psychological Stress And Anxiety Among Postoperative Gastric Cancer Patients

Objectives:Based on the investigation and research of patients with gastric cancer after surgery,to collect and gather all personal data,mental health survey data.To apply statistical methods to analyze the distribution and characteristics of psychological status and anxiety of patients with gastric cancer after operation.To provide a reliable basis prevention and treatment of postoperative gastric cancer patients,reduce the mortality and improve the quality of life.Methods:In this study,patients underwent gastric cancer surgery were selected from the department of gastrointestinal nutrition and hernia of a 3A(Class Three/Grade A)hospital of Jilin,Changchun from Jau 2018 to July 2018.The self-designed demographic questionnaire(age,gender,educational level,family living surgery,family history,whether or not patients have knew the true condition of themselves,psychological status etc.)was applied.Kessler10(K10),Self-Rating Anxiety Scale(SAS)review questionnaire were used.All investigators have the medical and nursing teacher qualification.To collect,organize,analyze all the questionnaires according to the data collected from patients,and set up the general database using excel.Data analysis was peformed by SPSS 21.0.Categorical variables were described as proportion and rate.Chi-square test was used to compare betweem groups.Single and multi factor logistic regression analysis was used for analysis.Results:(1)A total of 200 cases were in this survey.Among the patients,124(62.0%)were males and 76(38.0%)were females.31 people(15.5%)were under 40 years old,74 people(37.0%)were 40-65 years old,and 95 people(47.0%)were more than 65 years old which was the largest proportion.The cultural level was as follows: 88 patients(44%)with primary school or below cultural level,76 patients(38%)with senior high school cultural level,which accounted for the largest proportion,and 36 people(18%)with high school education or above.There were 55(27.5%)patients whose family monthly average income less than 1500,84(42.0%)between 1500 and 3000,and 61(30.5%)greater than 3000.183 people(91.5%)were married.Medical insurance types were as follows: 190(95.0%)people with medical insurance and 10(5%)people at their own expense.There were 107(3.5%)people with good sleep and 93(46.5%)people with poor sleep.133 people(66.5%)knowed their disease condition before surgery.(2)In the total 200 patients who received the survey,the average score of K10 was 26.91±9.16,the lowest score was 11,and the highest score was 47.The lower the score was,the lower the risk of psychological stress was.K10?25 was defined as high psychological state.There were 94 patients with K10 ?25(47.0%).Single factor logistic regression revealed that there were no significant differences between gender,age,educational level,family monthly average income,work status,marital status,payment of medical expenses,sleep status,disease condition awareness and high psychological state(P>0.05).Only family history was associated with high psychological state(P=0.032).High psychological status was set as dependent variable,and family history as independent variable adjused by age and sex in logistic regression model.The results showed that patients with family history was 6.977 times than that without family history,suggesting that patients with family history was an influencing factor.(3)In the total 200 patients,the average score of SAS was 51.62 ± 6.94,the lowest score was 11,and the highest score was 68.SAS?50 was defined as anxiety.The lower the score was,the lower the risk of anxiety was.There were 91 patients with SAS?50(45.5%).Single factor logistic regression revealed that there were no significant differences between gender,age,educational level,work status,marital status,payment of medical expenses,family history and anxiety(P>0.05).Sleep status,family monthly average income and disease condition awareness were associated with anxiety(P<0.05).Anxiety was set as dependent variable,and family income,sleep status and disease condition awareness as independent variable adjused by age and sex in logistic regression model.The results showed that patients with worse sleep status was 3.922 times than patients with better sleep status,suggesting that patients have worse sleep status was an influencing factor.Patients family monthly average income less than 1500 was 3.058 and 2.755 times than that between 1500 and 3000,and greater than 3000,suggesting that lower family monthly average income was an influencing factor.Patients with awareness of disease condition was 2.535 times than patients with unawareness of disease condition,suggesting that patients with awareness of disease condition was an influencing factor.Females was 4.356 times than males,suggesting that females was an influencing factor.Conclusion:Postoperative patients with gastric cancer have high psychological stress and anxiety,which is higher than that of the general population.Postoperative psychological stress of patients with gastric cancer is mainly affected by family history,and anxiety is mainly affected by sleep quality,monthly family income per capita,awareness of disease condition and gender.
