Explore The Relationship Between The Chronic Periodontitis And The Depression-Anxiety Psychological Factor, The Expression Of IL-8 In The Gingival Crevicular Fluid

Objective: Explore the relationship between the chronic periodontitis and the depression-anxiety psychological factors, as well as the expression of IL-8 in the gingival crevicular fluid (GCF).Method: The selected case group involved 31 patients randomly. Matching the case group with age and gender, The selected control group involved 29 subjects from the volunteers and the people taking health examination. In order to assess the depression-anxiety psychological index, the subjects filled the questionnaire concerning the demographic and socioeconomic information and the oral hygiene habit, as well as the Center for Epidemiologic Studies Depression Scale (CES-D)and the Self-Rating Anxiety Scale(ASA), then collect the gingival crevicular fluid(GCF). CI, BOP, PD, CAL, FI and tooth mobility were assessed at six sites per tooth (mesiobuccal, buccal, distobuccal, mesiolingual, lingual and distolingual) of all erupted teeth, using a manual periodontal probe. The amount of IL-8 in the GCF is measured via the radio-immunoassay method. All the gathered data were analyzed by the t-text, x~2 test and multivariable Logistic step wise analysis via the software of the SPSS 15.0. Results:1.The mean CAL of the case group is 0.46±0.16, while the mean CAL of the moderate periodontitis, the high periodontitis and the severe .periodontitis are 2.84±0.12, 3.51±0.34 and 4.71±0.51, respectively. The difference between every two group of the case group, the moderate periodontitis, the high periodontitis and the severe periodontitis has statistical significance, P< 0.001.2. The Multivariate Logistic analysis of the relationship between the chronic periodontitis and the depression-anxiety psychological factors demonstrated that the depression psychological factor got B=2.301, OR=9.988. while the optimistic coping style got B= -5.174, OR=0.006 in the equation of the regression.3. The amount of IL-8 in the GCF of the case group is 0.12±0.04, while the amount of IL-8 in the GCF of the moderate periodontitis, the high periodontitis and the severe periodontitis are 0.10±0.03, 0.20±0.01 and 0.22±0.02,respectively. The difference between the high periodontitis, the severe periodontitis and the moderate periodontitis ,control group is statistically significant, P<0.001.4. The linear regression analysis of the relationship between the amount of IL-8 in the GCF and the anxiety psychological factor get F=7.330 P=0.009 in the equation. The linear regression analysis of the relationship between the amount of IL-8 in the GCF and the depressive psychological factor get F=7.250 P=0.010 in the equation.Conclusion:1. The depression psychological factor could promote the progression of the chronic periodontitis. Besides, the optimistic coping style could prohibit the progression of the chronic periodontitis.2. Among the chronic periodontitis group, the amount of IL-8 of the GCF in the high periodontitis and severe periodontitis is much higher than that of the moderate periodontitis and the control group.3. Compared with the depression psychological factor, the anxiety psychological factor has more closer relationship with the amount of IL-8 within the chronic periodontitis.
