Curative Effect Of Kuh-seng And Pulsa-tilla Chinensis Mixture Retention Enema Therapy On The Treatment Of Damp-heattype Ulcerative Colitis And Its Role In A-djusting IL-12/IL-13 Ratio

Objectives: To tentatively study the possible correlation between the treatment of ulcerative c olitis(damp-heat syndrome)through Shenbai Mixture retention enema therapy and adju stment of quantity ratio of Th1/Th2 cells,which can be realized through the observati on of the clinical effect of Shenbai Mixture retention enema therapy on the treatment of ulcerative colitis(damp-heat syndrome)as well as content measurement of interleuk in-12(IL-12)and interleukin-13(IL-13)in blood serum via enzyme linked immunosorben t assay,and hopefully to provide the theoretic basis for clinical application of Shenbai mixture retention enema therapy.Methods: 1 72 patients with hygrothermal ulcerative colitis were randomly into treatment group and control group with 36 cases in each group.The treatment group was treated with Shenbai Mixture retention enema,while the control group was treated with mesalazine retention enema,the treatment lasted for one mouth.The relapse rate was followed up in 1 month after treatment.The changes of TCM syndromes and mucosal integral under electronic colonoscopy before and after treatment were compared to evaluate the clinical effect of Shenbai Mixture in treatment of hygrothermal ulcerative colitis.2 The contents of interleukin-12 and 13(IL-12 and 13)in serum were determined by enzyme linked immuno-sorbent assay(ELISA)to analyze the change of IL-12/IL-13.Results: 1 After treatment,the TCM syndromes integral and electronic colonoscopy scores were significantly improved both in the treatment and control group compared with those before treatment(P<0.05).In the aspects of purulent blood mucus,diarrhea,burning sensation and scanty dark urine,Shenbai Mixture group exhibited significant improvement compared with control group(P < 0.05),while in abdominal pain,tenesmus,fever and the changes of mucous,there was no significant difference between the two groups.2 Before treatment compared to after treatment,the decreased IL-12,elevated IL-13 and reduced IL-12/IL-13 ratio were observed in both groups after treatment compared with those before treatment(P < 0.05).But there is no difference between the two groups.3 The total effectiveness of clinical comprehensive curative effect in treatment group was 88.23%,while 81.81% of control group,with therapeutic equivalence(P>0.05).4 The total effectiveness of TCM syndromes curative effect in treatment group was in treatment group was 79.4%,while 75.6% of control group,with therapeutic equivalence(P>0.05).5 Observe the rate of recurrence at 1 month after treatment,Shenbai Decoction group was better than 5-ASA group.6 There was no adverse reaction in the two groups.Conclusion: In the aspect of total effectiveness,Shenbai Mixture retention enema is equal with 5-ASA,and superior to the control group in improving the purulent blood mucus,diarrhea,anal burning sensation,scanty dark urine.The possible mechanism of action maybe related to the ratio of Th1/Th2 cells.
