The Clinical Study On Cervical Spondylosis And Neck Pain Treated By Different Acupuncture Methods In The Reflecting Point Of Meridian Pathological Changes

ObjectiveThe purpose of this study was to verify the effectiveness of acupuncture on cervical spondylosis and neck pain at the reflecting point of meridian pathological changes.And the distribution of the reflecting point of meridian pathological changes associated with cervical spondylosis and neck pain was summarized.MethodsThe study included 108 patients according to the standard and randomized them to 3 groups: the treatment group(the Qianci-acupuncture group of the reflecting point of meridian pathological changes,referred to as the reflecting acupoint-Qianci group),the control group 1(the common acupuncture group)and the control group 2(the common acupuncture group of the reflecting point of meridian pathological changes,referred to as the reflecting acupoint-Zhici group),36 cases in each group.Before treatment,the reflecting point of meridian pathological changes was examined and recorded.For the acupoint selection,the reflecting acupoint-Qianci group will use the meridian diagnostics to figure out the reflecting point of meridian pathological changes which included the following situation: the complaint area,skin color,pore density,skin depression or skin mass,the abnormal skin temperature,the tender point and the subcutaneous nodules.Operation methods: patient sitting position,after the disinfection,insert needles into the reflecting point with 10-25 degree oblique and 0.3-0.5 inch deep,patients' neck movement is required at the same time,then adjust the direction and angle of acupuncture until the pain is released.For the reflecting acupoint-Zhici group,the same acupiont selection as the treatment group,and insert needles without any action.For the common acupuncture group,acupoint selection: Tianzhu,Fengchi,Quchi,Waiguan,Cervical Jiaji,Ashi,and insert needles without any action.All cases were treated daily or every other day,1 week at least 4 times for a total of 2 weeks.The curative effect of the three groups were evaluated for 4 times by the Northwick Park neck pain scale(NPQ)and McGill Pain Questionnaire(MPQ)and soft tissue tension(including D value and S value): pre-therapy,after 1 week therapy,post-therapy and follow-up of one month.Limited by objective conditions,soft tissue tension measurement does not require in the follow-up of one month.The overall efficacy evaluation is evaluated at the end of treatment.Use the SPSS 18.0 to set up database,and analyse the data collected with repeated ANOVA,rank sum test,analysis of variance(ANOVA)and Chi-square test.The test level is 0.05.When the three groups are compared,the inspection level is adjusted to ?=0.05/3?0.0167.Results(?)For the general baseline situation,there is no significant statistical difference among the three groups,which includes the gender,age,disease duration,disease classification,the scores of NPQ and MPQ before treatment,the D value and S value before treatment(P>0.05),showing a balance of baseline characteristics of the three groups and therefore they are comparable.(?)The distribution of the reflecting point 1.In this clinical study,the disease is located in the meridians and the meridian of muscle region(Jingjin),through the meridian diagnostics shows that the tender point(100%)and the subcutaneous nodules(69.44%)of the most frequent,more than half of the patients,and about 20% of patients can be found in superficial-vessel or skin depression or skin mass or pore density of abnormalities,and skin color of abnormalities,skin temperature changes or numbness appear in this study the least frequency,less than 3%.2.Further investigation of the tender point of patients with cervical spondylosis and neck pain in this study shows that the highest frequency of occurrence is the neck-pillow line(95.37%),followed by the supraspinous fossa(near Jangjing acupoint,84.26%),the side of C2 spinous process(83.33%),the inside of the scapula on the inside edge(81.48%).The side of C3 and C5 spinous process and sternocleidomastoid are appear more than 60%.The frequency of tender points on C4,C6 and C7 side of the spinous process also exceeded 50%.The meridians in which the tender points are distributed mainly include Du Meridian,Bladder Meridian of Foot-Taiyang,small Intestine Meridian of Hand-Taiyang,Sanjiao Meridian of Hand-Shaoyang and Gallbladder Meridian of Foot-Shaoyang.(?)Effect analysis: 1.Subjective evaluation: including NPQ neck pain scale and McGill Pain Questionnaire(MPQ).Comparison within each group: The values of NPQ scores and MPQ scores decreased after treatment in each group compared with the values measured before treatment,and after the correction group comparison The difference was statistically significant(P <0.001),suggesting that each group of acupuncture methods in alleviating neck pain,improve soft tissue tension have a real effect.Comparison between groups: After 1 week of treatment and at the end of treatment: the score difference between the treatment group and the other two groups was statistically significant(P <0.0167),and there was no significant difference between the two control groups(P> 0.0167),which indicated that the early and final effect of the treatment group was the best,while the early and final effects were the same in the two control groups.In the follow-up of one month: the differences among the groups were statistically significant(P <0.0167),suggesting that the long-term effect in the treatment group is better than the other two groups,it has a long-term solid effect and its recurrence rate is lower,while the control group 2 is superior to the other group.2.Objective comment: including D value and S value.Comparison within each group:The D value and S value increased after treatment in each group compared with the values measured before treatment,,and after the correction group comparison The difference was statistically significant(P <0.001),suggesting that each group of acupuncture methods in alleviating neck pain,improve soft tissue tension have a real effect.Comparison between groups: After 1 week of treatment and at the end of treatment: there was significant difference in value between the treatment group and the control groups(P <0.0167 or P <0.05),and there was no significant difference between the control groups(P> 0.0167 or P >0.05),suggesting that the treatment group to improve the effect of soft tissue tension is clear and effective,while the two control groups has the same effect that worse than the treatment groups.3.Comparison of the overall effect of the three groups after treatment: The treatment group contributes a curative rate as high as 86.11%.The control group 2 contributes a curative rate as high as 66.67%.The control group 1 contributes a curative rate as high as 52.78%.Comparison of the overall effect of the three groups shows that the overall effect of the reflecting acupointQianci treatment is the best,and the other two methods is similar.Conclusions(?)The reflecting acupoint-Qianci treatment to relieve cervical spondylosis and neck pain,and improve soft tissue tension have real effect.(?)The effect of the reflecting acupoint-Qianci treatment on cervical spondylosis and neck pain is much better than the other two methods,and the efficacy is consolidated.Otherwise,the the common acupuncture group and the reflecting acupoint-Zhici group have similar efficacy,whereas the effect of the follow-up one month of the latter one is better than the former one..(?)Patients with neck pain in cervical spondylosis all have the reflecting point of meridian pathological changes.While the tender point and the subcutaneous nodules is most common,and the common tender point including the neck-pillow line,the side of cervical spinous process,sternocleidomastoid,the inside of the scapula on the inside edge and the supraspinous fossa.The meridians in which the tender points are distributed mainly include Du Meridian,Bladder Meridian of Foot-Taiyang,small Intestine Meridian of Hand-Taiyang,Sanjiao Meridian of Hand-Shaoyang and Gallbladder Meridian of Foot-Shaoyang.
