Objective:To analyze the TCM syndrome distribution of the senile peptic ulcer, and its correlations with Helicobacter pylori infection and diagnosis of gastroscope. Methods:A total of 321 cases of diagnosis and treatment data of patients with peptic ulcer in the elderly were collected and organized, including TCM diagnosis, the results of gastroscopy and Helicobacter pylori detection, etc,TCM typing and using the statistical analysis to explore the correlations.Results:1.In the senile peptic ulcer, the ratio of male to female is 1.94:1,types of ulcers dominated by duodenal ulcers(178cases),helicobacter pylori positive rate 48.29%,ulcer staging is mainly active(218 cases),common complications included complications of gastrointestinal bleeding in 130 cases(40.50%),pathological intestinal metaplasia in 47 cases(14.64%),clinical complaints of symptoms such as arrhythmic pain and black stool and other atypical symptoms, taking non-steroidal drugs 30 cases.2.The TCM syndrome of senile peptic ulcer including spleen-stomach damp-heat syndrome, stagnated heat in liver and stomach syndrome, spleen and stomach deficiency-cold syndrome, deficiency of heart and spleen syndrome, incoordination between the liver and stomach syndrome, cold heat complicated syndrome, deficiency of stomach yin syndrome. There is a statistical difference between the spleen-stomach damp-heat syndrome, stagnated heat in liver and stomach syndrome, spleen and stomach deficiency-cold syndrome and other syndrome types(P<0.05).The excess syndrome is more common than the deficiency syndrome(P<0.05).There is no statistical difference between TCM syndromes with the detection of Helicobacter pylori, the classification of ulcer diseases, ulcer stage, gastric mucosal appearance, disease examination results, and the use of non-steroidal anti-inflammatory drugs(P>0.05).There are statistical differences in the complications and complaints of ulcers among syndromes of TCM(P<0.05),94.12% of patients with deficiency of heart and spleen syndrome combined with gastrointestinal hemorrhage and complained of hematemesis and black stool were significantly higher than other syndromes. Conclusion:the senile peptic ulcer is dominated by duodenal ulcer, clinical symptoms are not typical, and the rate of combined gastrointestinal bleeding is high,The positive rate of Helicobacter pylori detected by urea breath test is low.TCM syndrome differentiation is mainly based on spleen-stomach damp-heat syndrome, stagnated heat in liver and stomach syndrome, spleen and stomach deficiency-cold syndrome followed, deficiency of heart and spleen syndrome, incoordination between the liver and stomach syndrome, cold heat complicated syndrome, deficiency of stomach yin syndrome is rare. There are no correlation between TCM syndromes and Helicobacter pylori, ulcer classification, ulcer staging, gastric mucosal appearance, medical examination results, and use of non-steroidal anti-inflammatory drugs. There is a certain correlation between TCM syndromes and ulcer complications and complaint symptoms, heart and spleen deficiency syndrome is dominated by ulcers with gastrointestinal bleeding, chief complaints of hematemesis and black stool.
