| Objective: 1.Through the collection of clinical case studies,summed up the Hp correlation of TCM syndrome type distribution characteristics of peptic ulcer,it is concluded that the most common witness for liver gastric stasis.2.To observe the clinical curative effect and combined out decoction combined with quadruple therapy on the disease of liver and stomach stagnation.Methods: 1.Choose our cadres digestive internal medicine outpatient and ward nearly five years(September 2011-September 2016)all been diagnosed with Hp correlation,470 cases of peptic ulcer patients,and on the basis of TCM tongue,pulse and symptomatology analysis to these patients classification,statistical analysis of the type of distribution.2.Choose 60 cases in October 2015-October 2016 in Anhui University of Traditional Chinese Medicine diagnosis and treatment of hospital outpatient and hospitalization for Hp related peptic ulcer of liver and stomach qi stagnation patients,randomly divided these patients into the treatment group and the control group of 30 cases,the control group was treated with esomeprazole + colloidalbismuthpectin + clarithromycin + levofloxacin,on the basis of the control group,the treatment group was given the combination of the decoction,and the course of treatment was 14 days.Two groups were treated with other drugs during the treatment,after treatment,the clinical symptoms of the two groups were observed,including the total score of clinical symptoms,symptom score,endoscopic ulcer healing,Hp eradication and liver and kidney function.Result: 1.Through clinical retrospective study found that 470 cases of peptic ulcer Hp correlation of TCM syndrome type distribution rule is: the spleen and stomach and qi stagnation syndrome of 170 cases(36.17%);Taste hot and damp card 129 cases,accounting for 27.45%;Spleen deficiency cold syndrome in 95 cases,accounting for20.21%;Stomach Yin deficiency syndrome 54 cases,accounting for 11.49%;Stasis resistance gastric collaterals 22 cases,accounting for 4.68%.Can be concluded that the proportion of liver gastric stasis syndrome is the largest.2.Joint out deep and remote soup decoction of the clinical curative effect observation combined western medicine treatment group are as follows: 2.1 Clinical studies have found that the treatment group USES the joint out deep and remote party and quadruple therapy of western medicine clinical total effective rate was 93.33%,clinical cure Hp correlation of 8 patients with peptic ulcer,11 cases were markedly effective,effective 9 cases,2 cases failed;While the control group USES the pure western medicine treatment the total effective rate was 70.00%,4 cases were cured,9 cases had marked effect,8 cases,9 cases of invalid.Through statistical analysis to the following conclusion: P < 0.05,comparing the two groups statistically significant,the clinical curative effect of treatment group is better than the control group.2.2 The comparison of two groups of patients in terms of symptom integral:Symptoms of treatment group and control group in total scores and clinical symptoms of the treatment before comparison shows P > 0.05),there was no statistical difference,with clinical comparability.The total score of symptoms,the treatment group and the control group after treatment,the treatment group before and after treatment were compared,the test were statistically significant(P<0.05),the results showed that the clinical efficacy of the treatment group was significantly better than that of the control group.In the clinical symptom score,the treatment group and the control group were treated with the noisy one(P<0.05),and the two groups were analyzed by P>0.05,indicating that there was no significant difference between the two groups in the treatment of noise.Two groups of stomach pain,heartburn,belching,anorexia,distension,dry mouth bitter disease treatment results showed that the treatment group and the control group have been treated better clinical efficacy(P<0.05),and the two groups had statistical significance(P<0.05),the treatment group of the clinical symptoms in addition to the noisy outside were better than the control group.2.3 Two groups of patients in the endoscopic ulcer healing,the total efficiency of the treatment group was 83.33%,8 cases were cured,10 cases markedly effective in 7 cases,5 cases were ineffective;the total efficiency of the control group was 60%,4 cases were cured,6 cases markedly effective 8 cases,invalid 12 cases,the results of the two groups statistical analysis.There was statistical significance(P<0.05).The results showed that the treatment group was more effective in promoting the healing of ulcer.2.4 The two groups in the case of eradication of Helicobacter pylori,treatment group and control group in the 14C-UBT values were decreased after treatment,the two groups before and after treatment were statistically significant(P<0.05),while the two group after treatment between two groups of P<0.05,indicating that the treatment group on the eradication rate of Hp is significantly better than the control group.After treatment,the positive rate was 10% in the treatment group,positive in 3 cases and negative in the control group,the positive rate was 26.70%,positive in 8 cases,negative in 22 cases,the test group was statistically significant()(P<0.05).It can be concluded that the clinical clearance rate of Hp in the treatment group is better than that in the control group,which is worthy of clinical application.2.5 Safety index: Chinese medicine treatment group no patients discomfort reaction in patients in the control group there were 3 cases of anorexia,fatigue has appeared in 2 cases,accounting for 16.70% of the total cases,due to a lesser extent,life after diet and sleep after adjustment were relieved.Blood routine,urine routine,stool routine + occult blood and ECG,liver and kidney function of two groups after treatment showed no abnormal.The incidence of adverse reactions of the two groups was statistically significant(P<0.05),which showed that the clinical use of Chinese medicine treatment group was higher than that of the control group.Conclusion: 1.In September 2011-September 2016’s collection of 470 patients,Hp correlation peptic ulcer patients with hepatic gastric stasis of the most common type.2.Through joint out deep and remote parties joint quadruple therapy of western medicine treatment of peptic ulcer gastric stasis type liver Hp correlation observation,can be concluded that: the method can effectively relieve clinical symptoms of liver in patients with gastric stasis type,promote the healing of ulcer under gastroscope,have the effect of repair of gastric mucosa,joint out deep and remote soup combined quadruple therapy can effectively kill Hp,and relieve the patient’s clinical adverse reactions. |
