| Objective:The purpose of this study was to discuss the efficacy and complications of cannulated screws internal fixation and external plaster fixation in the treatment of fifth metatarsal base avulsion fractures in adults.Methods:A retrospective study was conducted to collect patients with avulsion fractures of the fifth metatarsal base in our hospital from May 2015 to May 2017.A total of 58 patients were enrolled in this study.Among the 58 cases,32 cases were treated with open reduction and cannulated screws fixation,and 26 cases were treated with manual reduction and plaster fixation.The full weight-bearing time,fracture healing time,prognosis and post-treatment complications were compared between the two groups.Prognosis was based on the AOFAS mid-foot scoring system and visual analogue scale(VAS).Results:All 58 patients were followed for an average of 18 months(8-24 months).In the Surgical and conservative treatment groups,the full weight-bearing time was 6.16±1.17 and 7.42±0.90 weeks,fracture healing time was 10.06±1.08 and 11.85±1.26 weeks,and the AOFAS mid-foot score was 88.35±2.91 and 90.5±2.24 points,the difference was statistically significant(P>0.05);The VAS score of the surgical and conservative group were 1.04 ± 0.53 and 0.94 ± 0.35 points,the difference was not statisticallysignificant(P<0.05).There were significant differences in complications between the two groups,the proportion of sural nerve stimulation was significantly higher in the surgical treatment group than the conservative treatment group;the proportion of malunion,delayed healing,non-union of the conservative treatment group was significantly higher than that of the surgical treatment group.Conclusion:Surgical treatment of the fifth metatarsal base avulsion fracture is a safe and effective method,which can achieve good functional effects,and can also reduce the occurrence of malunion,delayed healing or non-union,but the incidence of sural nerve stimulation is higher. |
