Objective:Compared a medial parapatellar approach incision and cannulated screw fixation, arthroscopic cannulated screw fixation and arthroscopy wire fixation of the three different surgical methods of anterior cruciate ligament ltbia avulsion fractures, and analysis of curative effect. To provide reference for clinical treatment. Methods:In our hospital from October 2011 to June 2015 54 cases of avulsion fracture of tibial spine were analyzed retrospectively. According to the different operation mode. The patients were divided into incision and cannulated screw group(22 cases), arthroscopic cannulated screw group(11 cases) and arthroscopic wire group(21 cases). All patients were followed up for at least 6 months. The function of knee joint was evaluated by Lysholm, IKDC and Tegner scoring system. The age, sex, type of fracture, injury mechanism, injury to operation time and follow-up time of the three groups were not statistically significant(P>0.05). Results:All of the 54 patients were followed up, and the traffic injuries accounted for 76%. After 2 months, Lysholm score of the three groups respectively:(84.06±0.88),(90.13±1.42),(90.20±3.16), IKDC subjective function score were:(86.09±3.58),(90.13 ±3.20),(90.63±3.68). The Tegner score:(4.0±0.24),(4.5±0.22),(4.3±0.35), group differences were statistically significant(P < 0.05). Six months after surgery, Lysholm score of the three groups respectively:(93.56±1.22),(94.59±1.28),(93.98±116), IKDC subjective function score were:(92.76±1.12),(93.95±1.50),(93.27±1.38). The Tegner score:(8.0±0.28),(8.2±0.39),(8.2±0.33), the difference between the three groups no statistical significance(P > 0.05). Conclusion 1. The treatment of avulsion fracture of tibial spine, the early minimally invasive group (arthroscopic hollow screw group and arthroscopic wire group) than the open screw group has a certain advantage, the recent clinical efficacy is better. 2. Minimally invasive group and open group in the treatment of tibial spine avulsion fracture and postoperative metaphase knee joint function rehabilitation ideal, have high value in clinical application, each with advantages and preoperative should strictly control the indications for surgery, long-term effect needs further research and the best operation method needs to be improved.
