Purpose : Through the comparative analysis of conservative treatment and surgical treatment of elderly of type C fractures of the distal part of the radius of the wrist joint function recovery and complications before and after the situation, to study the treatment elderly of type C fractures of the distal part of the radius of the choice.Material and method:A retrospective analysis of 52 cases of type C fracture patients from November 2013 to June 2015 in Shenyang City Orthopaedic Hospital admitted elderly distal radius. Conservative group(Manipulation of small splint external fixation) 30 cases, surgery group(Open reduction and internal fixation) 22 cases. Before measuring record is reset by X-ray, after clinical healing wrist volar tilt, ulnar deviation and distal radius length. After 6 months, according to Cooney wrist function scoring system were two groups of patients with wrist function scores, and complication rates of the two treatments were statistically analyzed and compared.Results: 1.After treatment, the X-ray contrast performance by not statistically significant. Two fracture healing time: the conservative group(11.43 ± 1.305) weeks, the surgery group(11.86 ± 1.583) weeks. T test, P = 0.477> 0.05, not statistically significant. 2.Assessing volar tilt, ulnar deviation and radial extent of the degree of recovery shortening by t test, P <0.05, statistically significant. Compare palmar angle loss, loss of ulnar, radial length loss extent, by t test, P <0.05, statistically significant. 3.Cooney wrist score at last follow-up results: Conservative Group: excellent in 4 cases, good in 16 cases, 8 cases acceptable, poor in 2 cases;Surgery Group: excellent in 7 cases, good in 8 cases, acceptable in 5 cases, and poor in 2 cases.Comparison results by X2 test, three groups P < 0.05, with statistical significance.Conclusion: 1.Clinical conservative treatment and surgical treatment of senile distal radius C fractures healing time is insignificant, the incidence of complications after treatment, no significantdifference. 2.Assessing volar tilt, ulnar deviation and radial extent of the degree of recovery shortening contrast surgery group were better than the conservative group, and the palmar angle, ulnar deviation, radial length of the surgery group were less than the extent of the loss of the conservative group. 3.Elderly of type C distal radius fractures surgery is superior to conservative treatment. However, in clinical work should be based on the specific circumstances of the patient, selective use.
