Comparison Of Curative Effect Between Conservative Treatment Versus Surgical Treatment In Distal Radius Fracture:A Meta-analysis

Objective:To collect the articles that use the surgical treatment or conservative treatment for distal radius fracture and evaluate the curative effect by a Meta-analysis..Method:By using the method of system review, searching the studies from EMBASE, Pub Med,the Cochrane Library, Ovid, MEDLINE, CNKI, Wanfang, and search those who use both conservative and surgical treatment methods to treat distal radius fractures and evaluate the curative effect of randomized controlled studies by manual retrieval;After extraction of literature data and quality evaluation, use Revman5.2 Meta analysis software to compare the two groups at the time of last follow-up PRWE score satisfaction rate, the good rate of Gartland – Werley score, complication rates, radial height, parlmar tilt and ulnar inclination recovery outcome indexes.According to the demand of Cochrance network with P≤0.10 for the standards of heterogeneity inspection; For no heterogeneity data using FEM; If there is heterogeneity using REF. Dichotomous data were analyzed using the odds ratio(OR) measure and its 95% CI, whereas continuous variable was analysed using the weighted mean differences(WMD) with its 95% confidence interval(CI).Results:According to into the exclusion criteria, we identified a total of 11 articles and a total of1091 patients which contains 637 people in conservative treatment group and 454 people in surgical treatment group. After extract the data and meta analysis, the results hint: the satisfaction of PRWE score(OR=0.82,95%CI : 0.49~1.38, p=0.46), the incidence of complications(OR = 0.78, 95% CI: 0.27-1.88, p = 0.50) and radial high recovery(MD = 0.51,95% CI:-1.62~2.93, p = 0.68) has no statistical significance between the two groups; while there was statistical significance in the good rate of Gartland-Werley score(OR = 0.41, 95%CI: 0.27~0.61, p < 0.05), ulnar inclination recovery outcome(MD =- 4.08, 95% CI:-7.78 ~-0.38, p < 0.05) and parlmar tilt recovery outcome(MD = 10.76, 95% CI:-12.90 ~-8.62, p <0.05).Conclusions:Surgical treatment is superior to conservative treatment in combination of objective and subjective factors in the overall treatment. However, there was no significant difference from the subjective effect on the patient, so choose surgery or not should respect the patients’ subjective requirements.
