Clinical Study Of Conservative Treatment And Open Reduction And Internal Fixation For Treatment On Proximal Humerus Fractures Of Neer Type Ⅲ

Objective:The work aimed to study and compare the clinical effect between conservative treatment and surgical treatment on proximal humerus fractures of Neer type III.Methods:This study retrospectively analyzed the clinical diagnosis of 97 patients,with the income from the Department of orthopedics of Hubei Provincial Traditional Chinese Medical Hospital during October 2013 to October 2015,and all of them had the clinical diagnosis of "proximal humerus fracture".And all the case of the study were completed.Because of different treatment,two groups were divided.And Group A was for the surgical treatment while group B was for the conservative one.Those were compared between the two groups that changing in the treatment of ipsilateral shoulders’ pain,function and so on.SPSS21 version of the software was used to analysis the dates.Results:1.The two groups’ pain score were compared.Before treatment,the comparison of two groups had no significant difference(P=0.919>0.05).Immediately after treatment,the comparison of two grou-ps had significant difference(P=0.000<0.05).First days after treatment,the comparison of two groups had significant diff-erence(P=0.000<0.05).Five days later after treatment,the com-parison of two groups had significant difference(P=0.000<0.05).4 weeks later,the comparison of two groups had no significant difference(P=0.883>0.05).2.The two groups’ limb swelling grade were compared.Before tr-eatment,the comparison of two groups had no significant diff-erence(P=0.954>0.05).Immediately after treatment,the compar-ison of two groups had no significant difference(P=0.865>0.05).First days after treatment,the comparison of two groups had significant difference(P=0.000<0.05).Five days later aft-er treatment,the comparison of two groups had significant diff-erence(P=0.000<0.05).4 weeks later,the comparison of two gro-ups had no significant difference(P=0.678>0.05).3.The two groups’ reduction of the fracture were compared by the X-ray film.After the treatment,the comparison of two groups had significant difference(P=0.000<0.05).4.The two groups’ Neer classification of injured shoulders were compared.After the treatment within 3 days,the comparison of two groups had no significant difference(P=0.203>0.05).4 wee-ks later after the treatment,the comparison of two groups had significant difference(P=0.000<0.05).12 weeks later after the treatment,the comparison of two groups had significant difference(P=0.001<0.05).24 weeks later,the comparison of two groups had no significant difference(P=0.633>0.05).5.24 weeks later after the treatment,the two groups,Neer cl-assification between the injured shoulders and the other one were compared.There was no significant difference(P=0.986>0.05)between two groups’ uninjured shoulders.There was no si-gnificant difference(P=0.633>0.05)between two groups’injured shoulders.Discussion:Through this researching work,the traditional Ch-inese medicine conservative treatment of Neer type III fractu-res of the proximal humerus,in some degree,is worthy of apply-ing in clinical work.
