Advantages And Disadvantages Of Laparoscopic And Open IPOM In Abdominal Incisional Hernia Application

Objective:Abdominal wall incision is one of the complications after abdominal surgery.The only effective treatment is surgical repair.With the development of minimally invasive techniques,laparoscopic instruments and artificial patch materials,laparosco pic abdominal wall Incision herniorrhaphy is gradually becoming the treatment of incision hernia.In this study,by observing the clinical efficacy of laparoscopic an d open IPOM,the advantages of laparoscopic incision hernia repair in the treatme nt of abdominal incision hernia were analyzed.Methods:The clinical data of 76 patients with abdominal incision hernia treated in our hospital from January 2012 to July 2017 were retrospectively analyzed.Accordin g to the operation methods,they were divided into open IPOM group(n=45)and LVHR group(n=31)Observe and compare indexes such as operation time,intrao perative blood loss,postoperative hospital stay,total hospitalization costs,postoper ative pain,early postoperative complications,and postoperative recurrence rate.Results:Compared with the relevant indexes of operation,the LVHR group had less intrao perative blood loss and postoperative hospital stay than the IPOM group(P<0.05).The LVHR group had more operation time and hospitalization costs than the IP OM group(P<0.05).There was no significant difference in the recurrence rate bet ween the two groups(P>0.05);compared with the surgical complications: The inc idence of postoperative wound infection,abdominal pain and intestinal obstruction was significantly lower in the LVHR group than in the IPOM group(P<0.05).T here was no significant difference between the LVHR group and the IPOM group in the incidence of postoperative intestinal injury and postoperative serum swellin g(P>0.05).Conclusion:Both open IPOM repair and LVHR repair are safe and effective.The LVHR group has less damage to the abdominal wall tissue and has the advantages of mild postoperative pain,shorter hospital stay,and quick recovery.It is an ideal su rgical technique for repairing abdominal incision hernia.
