Study Of Clinical Application Of Hybrid Technique In The Treatment Of The Huge Abdominal Incisional Hernia

Objective To analyze the clinical value of hybrid technique in the treatment of giant abdominal incisional hernia.Method Retrospective analysis of clinical data of 109 cases of huge abdominal incisional hernia admitted from Jan.2009 to Oct.2016 in the second hospital affiliated to Soochow University.All patients were followed up.Compared with open surgery(33 cases),hybrid technique(31 cases)and laparoscopic surgery(45 cases)for the treatment of giant abdominal incisional hernia related indicators of operational course and the incidence of postoperative complications,and follow-up of patients with postoperative recurrence rate.Results The meantime of operation in the open group[(110.97±53.43)min]was shortest and that in the laparoscopic group[(145.32±64.39)min] was longest.The length of incision in the hybrid group[(8.52±2.43)cm] was shorter than that in the open group[(15.55±2.86)cm].The amount of bleeding in the open group was largest and that in the laparoscopic group was least of operational course.Postoperative exhaust time and the hospitalization stay in the open group and hybrid group was longer than that in the laparoscopic group.Laparoscopic group was the least painful at 24 h after operation,and the pain was the most severe in 24 hours after operation.The degree of pain was lighter in the hybrid group than in the open group and laparoscopic group at one week after operation.The follow-up ranged from 6 to 36 months.sixteen cases developed complication in the open group,17 in the laparoscopic group and 7 in the hybrid group.One case developed recurrence in the open group,1 in the laparoscopic group and none in the hybrid group.Recurrence of 2 cases were cured by hybrid technique without re-treatment after treatment.Conclusion The application of hybrid technique in the treatment of giant abdominal incisional hernia with the advantages of laparoscopic surgery and open surgery,to overcome their shortcomings,has a certain advantage in the treatment of giant abdominal incisional hernia.
