Objective:The purpose of this study was to examine the changes of serum adiponectin,fibroblast growth factor 21 levels in type 2 diabetes mellitus and type 2 diabetes mellitus patients combined with CHD,and to investigate its clinical significance.Methods:According to the inclusion and exclusion criteria,85 subjects were selected,including type 2 diabetes group(n=29),type 2 diabetes mellitus combined with coronary heart disease group(n=29)and normal control group(n=27).The general information of all subjects were collected,and examine relevant indicators.Serum adiponectin and fibroblast growth factor 21(FGF21)levels were compared among each group,and the correlation between them and the indexes was analyzed.Results:(1)Compared with normal control group,age,body mass index(BMI)and triglyceride (TG)level in type 2 diabetes patients were significantly increased(P<0.05),while high density lipoprotein cholesterol level(HDL-C)was significantly decreased (P<0.05).(2)Serum FGF21 level was gradually increased in the normal control group,the type 2 diabetes group and the type 2 diabetes mellitus with coronary heart disease group,respectively(92[58~136],114[98~197],169[127~237]pg/ml,P<0.05).(3)Serum adiponectin level was gradually decreased in the normal control group,the type 2 diabetes group and the type 2 diabetes mellitus with coronary heart disease group,respectively(38524[29559~42589],19121[11608~30231],11857[4062~22022] ng/ml,P<0.05).(4)Spearman correlation analysis showed that serum FGF21 was positively correlated with triglyceride(r_s=0.355,P=0.001),and negatively correlated with high density lipoprotein cholesterol(r_s=-0.340,P=0.001).Serum adiponectin was positively correlated with high density lipoprotein cholesterol(r_s=0.482,P<0.001)and negatively correlated with age,body mass index and triglyceride(r_s=-0.278,P=0.01;r_s=-0.394,P<0.001;r_s=-0.425,P<0.001).(5)Compared with type 2 diabetes mellitus group,age,waist circumference,serum FGF21 level and the positive rate of family history of diabetes were increased in type 2 diabetes mellitus combined with CHD group;while serum adiponectin level was decreased.Further logistic regression analysis showed that serum FGF21 level, waist circumference and family history of diabetes were the risk factors for the patients with type 2 diabetes occur coronary heart disease.Conclusion:(1)Serum fibroblast growth factor 21 levels were significantly higher in type 2 diabetes mellitus with coronary heart disease,and it may be one of the influencing factors for the patients with type 2 diabetes occur coronary heart disease.(2)Hypoadiponectinemia may be involved in the occurrence and development of the patients with type 2 diabetes occur coronary heart disease.
