Objective : Cardiovascular disease morbidity is still arising in recent years,risk factors for atherosclerosis such as age,dyslipidemia,smoking,hypertension,diabetes,obesity,family history as well as other risk factors,such as type A personality,the Western diet,oral contraceptives are closely related to the development of coronary heart disease.Fibroblast growth factor 21 and adiponectin are newly discovered glycolipid metabolic regulators,The purpose of this study is to explore the association between fibroblast growth factor 21,adiponectin and coronary heart disease through the common traditional factors of coronary heart disease.Methods:Patients with recurring chest discomfort visiting the affiliated hospital of Qingdao university from June 2017 to November 2017 were included into the study.The subjects were examined by coronary angiography to observe the coronary artery of the epicardial coronary artery,according to the clinical manifestations,electrocardiographic characteristics and myocardial enzyme examination results,they were divided into acute myocardial infarction group,unstable angina pectoris group,coronary slow flow group and normal coronary artery group.The final determination of the sample size was 160 cases.Then using the enzyme-linked immune sorbent assay to detect the level of fibroblast growth factor 21 and adiponectin,biochemical indicators and C reactive protein levels were detected by automatic biochemical analyzer Hitachi 7600,BN2 specific protein of SIEMENS,respectively.Analyzing the correlation between fibroblast growth factor21,serum adiponectin and the serum indexes(High density lipoprotein cholesterol HDL?Low density lipoprotein cholesterol LDL ?Total cholesterol TC?Triglyceride TG ?apolipoprotein A?C reactive protein CRP),systolic blood pressure and Body Mass Index(BMI)respectively,then deduce the correlation with coronary heart disease finally.Results:The levels of systolic blood pressure,HDL,LDL,TC,CRP,FGF21 and adiponectin in patients with acute myocardial infarction group,unstable angina group coronary artery slow blood flow group and normal coronary artery group were significantly different in these groups(P < 0.05).Among them,systolic blood pressure level was only had obvious difference in acute myocardial infarction group and normal coronary artery group(130.86 ±8.54 VS 126.05± 5.85,P=0.007);compared with normal coronary artery group,HDL in acute myocardial infarction group and coronary slow flow of coronary artery group(1.25±0.16 VS 1.36 ± 0.17,P=0.003;1.26 ± 0.16 VS 1.36 ±0.17,P=0.008)had the difference;compared with normal coronary artery group,LDL in acute myocardial infarction group and unstable angina group(3.08 ±0.69 VS 2.70± 0.44,P=0.003;2.99±0.65 VS 2.70±0.44,P=0.02)had the difference;except in acute myocardial infarction group and unstable angina group(4.74±0.33 VS 4.79±0.29,P=0.447),coronary artery slow blood flow group and normal coronary artery group(4.56 ±0.33 VS 4.53±0.31,P=0.703),the level of TC had no significantly difference,but in other groups had significant difference with P < 0.05;compared with normal coronary artery group,CRP and FGF21 had significant difference in the myocardial infarction group,the unstable angina pectoris group,the coronary slow flow with P < 0.05,respectively,but in the other groups had no significant difference;adiponectin levels in unstable angina pectoris and coronary slow flow group(7.38±1.33 VS 7.18±1.64,P=0.503)had no significantly difference,however,in the rest other groups had significant difference with P < 0.05;Spearman rank correlation analysis showed that FGF21 was positively correlated with age,systolic blood pressure,LDL,TC,BMI and CRP,but negatively correlated with adiponectin;adiponectin was negatively correlated with age,systolic blood pressure,LDL,TC,BMI,CRP and FGF21,but positively correlated with HDL;after adjusted for age and BMI,partial correlation analysis showed that FGF21 was positively correlated with LDL,TC,CRP and systolic blood pressure(r=0.216,P=0.007;r=0.201,P=0.011;r=0.327,P <0.001;r=0.293,P < 0.001),but negatively correlated with adiponectin(r=-0.405,P <0.001),adiponectin was negatively correlated with FGF21,TC,CRP and systolic blood pressure(r=-0.405,P < 0.001;r=-0.173,P=0.03;r=-0.214,P=0.007;r=-0.243,P=0.002),positively correlated with HDL(r=0.247,P=0.002),but had no correlation with LDL(r=-0.130,P=0.104).Conclusion : FGF21 and adiponectin were associated with risk factors of cardiovascular disease such as age,BMI,LDL,TC,CRP,and systolic pressure.FGF21 level was elevated in cardiovascular diseases and was highest in acute myocardial infarction,while adiponectin level was declined and was the lowest level in acute myocardial infarction.The early changes of FGF21 and adiponectin can be used as an early monitoring indicator for coronary heart disease;There is a negative correlation between the adiponectin and FGF21.It is possible that the protective effect of FGF21 on cardiovascular disease is achieved by the adiponectin as downstream factor,and the FGF21 level is compensatory elevated but has not fully functioned its benefit under the low level of adiponectin,which eventually leads to the occurrence of coronary heart disease.Therefore,there is a correlation between FGF21,adiponectin and the occurrence and development of coronary heart disease.
