| Objective:To investigate the relationship between serum levels of adiponectin and coronary atherosclerotic heart disease (CHD) with diabetes mellitus (DM) severity, according to the adiponectin concentration of coronary heart disease group and group which divided by coronary angiography.Method:This study was conducted with 60 CHD Case (30 CHD Case,30 CHD with diabetes mellitus Case), diabetes mellitus (DM) 30 case, and 30 healthy adutts who were confirmed by coronary angiography (CAG). Exclusion criteria were with other heart diseases such as dilated cardiomyopathy, hypertrophic obstructive cardiomyopathy and rheumatic heart disease etc, hepatic function or renal function dysfunction and with infection, malignant tumor, hematological system or immunologic system diseases. Simultaneously to calculate the sum of integrals according to Gensini integral.Selected patients will be recorded its general, height,age, blood fat and CAG results. ELISA (Enzyme-linked immunosorbent assay,ELISA) was adopted to evaluate serum adiponectin concentrations.All data processed by spssl 1.5 package statistically.Result:①The serum levels of adiponectin of coronary atherosclerotic heart disease with diabetes mellitus group is 1.87±0.60 ng/ml,the serum levels of adiponectin of CHD group is 2.09±0.68 ng/ml, the serum levels of adiponectin of DM group is 2.26±0.95 ng/ml,the serum levels of adiponectin of normal control group is 2.76±0.38ng/ml, The serum levels of adiponectin of coronary atherosclerotic heart disease with diabetes mellitus group is lower than the control group, and the difference was statistically significant (P<0.001);②coronary atherosclerotic heart disease with diabetes mellitus and normal control patients are divided into four groups according to coronary lesions count:normal control group (30 cases), the serum levels of adiponectin is 2.76±0.38ng/ml,Single vessel lesion group (6 cases), the serum levels of adiponectin is 2.54±0.15ng/ml, double vessel lesion group (9 cases) and the serum levels of adiponectin is 2.06±0.18ng/ml, three lesions group (15cases), its serum levels of adiponectin is 1.42±0.20ng/ml. F value is 66.398, P<0.001, the differences between each group are statistically significant. The seurm levels of adiponectin of three lesion group is obviously lower than normal conyrols. The Gensini integral of each group is:normal control group 1.37±0.49, single lesion group 13.33±5.61,double lesion group 18.89+8.78, three lesion group 36.27±12.44, F value is 78.782, P<0.001, the differences between each group are statistically significant, three lesion groups Gensini integral is obviously higher than integral of single lesion and normal controls.③The correlation analysis of serum level of adiponectin and coronary lesions count, rs=-0.917 (P<0.001), has the remarkable statistical significant, adiponectin and coronary lesions count has a significant negative correlation.④Examing the correlation of adiponectin and Gensini integral with Pearson correlation analysis, the result is r=-0.813 (P<0.001), the differences are statistically, adiponectin and Gensini integral also shows significant negative correlation.Conclusion:It is observed that serum adiponectin concentration is closely relevant with coronary heart disease, and it reflects the extent of coronary atherosclerosis, coronary heart disease patients level of serum adiponectin is lower than non-CHD patients, and the more serious coronary artery lesion, the lower serum adiponectin concentrations. Therefore, serum adiponectin can be used as a nontraumatic indicators at a certain extent to guide clinical diagnosis and evaluate the coronary heart disease. |
