Clinical Study On The Treatment Effect Of Tong Bi Fumigation Decoction On Knee Osteoarthritis Of Wind-cold-dampness Retention Type

Objective: This paper focuses on observing the clinical curative effect and adverse reactions of Tong Bi Fumigation Decoction treating knee osteoarthritis with wind-colddampness retention type,and clearing the clinical advantages of Tong Bi Fumigation Decoction.Methods: There are 60 cases of knee osteoarthritis with wind-cold-dampness retention type were collected from May in 2016 to March in 2017,in the Department of Orthopedics,Affiliated Hospital of Gansu University of Traditional Chinese Medicine,inpatient department.The cases were randomly divided into two groups namely,the control group and the treatment group,30 cases in each group according to the method of random number table.The treatment group was given the hot compress on knee by Tong Bi Fumigation Decoction,2 times a day,every time 30 minutes,treatment for 2 weeks.The control group was given the Didofenac Diethylamine Gel brushing coated the knee,2 times a day,treatment for 2 weeks.Before the treatment,the patient's Western Ontario and McMaster Universities Arthritis Index(WOMAC)scale scores,TCM syndromes were recorded(pain,morning stiffness,swelling,bend to stretch disadvantage and heavy feeling)1 week after the treatment,2 weeks after the treatment,and 6 weeks after(after 4 weeks withdrawal),to evaluate clinical efficacy.SPSS22.0 was used for the statistical analysis of data.Results:Short curative effects analysis:(1)WOMAC scale score: after a week' treatment,the scores of the two groups of WOMAC scales were reduced,but there was no significant difference between the two groups(P>0.05).Compared with the former treatment,the scores of two groups on the WOMAC scale were significantly reduced after a week's treatment(P<0.05).After 2 weeks' treatment,the stiffness,functional activity scores and total scores of the WOMAC scale in the treatment group were significantly lower than those in the control group(P<0.05).The pain score in the treatment group had a decreasing trend,but there was no significant difference between the two groups(P>0.05).Compared with the treatment after,the scores of the two groups of WOMAC scales were significantly lower after 2 weeks' treatment(P<0.05).(2)TCM syndromes: after a week's treatment,the swelling scores and total scores of TCM syndromes in the treatment group were significantly lower than those inthe control group(P<0.05).The pain,morning stiffness,flexion and extension,and the heavy sensation scores of the TCM syndromes in the treatment group decreased,but it was no significant difference between the groups(P>0.05).Compared with the former treatment,the morning stiffness,bend to stretch disadvantage,and the scores of heavy feeling and total scores of the TCM syndromes of the treatment group were significantly improved after a week's treatment(P<0.05),the pain and swelling scores were not significantly improved(P>0.05).The morning stiffness,the score of heavy feeling,and the total score of the TCM syndromes in the control group were significantly improved(P<0.05),and the pain,swelling,and bend to stretch disadvantage scores were not significantly improved(P>0.05).After 2weeks' treatment,the morning stiffness scores and total scores of TCM syndromes in the treatment group were significantly lower than those in the control group(P<0.05).The pain,swelling,bend to stretch disadvantage,and heavy feeling scores of the TCM syndromes in the treatment group decreased,but it was no significant difference between the groups(P>0.05).Compared with a week's treatment,the scores of TCM syndromes in both groups were significantly lower after 2 weeks of treatment(P<0.05).Long term curative effects analysis:(1)WOMAC scale score: after 4 weeks' withdrawal,the scores of the WOMAC scale in the treatment group were significantly lower than those in the control group(P<0.05).Compared with the 2 weeks' treatment,the scores of the two groups of WOMAC scales were significantly lower than 4 weeks' withdrawal(P<0.05).(2)TCM syndromes: After 4 weeks' withdrawal,the TCM syndrome scores of the treatment group were significantly lower than those of the control group(P<0.05).Compared with 2weeks' treatment,the scores of TCM syndromes in the treatment group were improved significantly after 4 weeks' withdrawal(P<0.05);the pain scores and total scores of the TCM syndromes in the control group improved significantly(P<0.05),although the score of he morning stiffness,swelling,bend to stretch disadvantage of TCM syndromes decreased,it was no significant difference before and after the treatment(P>0.05).Through the comparison of the two groups of WOMAC scale scores and TCM syndrome scores at each follow-up point,it was indicated that the difference between the treatment group and the control group was statistically significant after a week' treatment,which shows that the Tong Bi Fumigation Decoction and Didofenac Diethylamine Gel were used.The effect time is similar.Through the comparison of curative effect,it was found that the effect of Tong Bi Fumigation Decoction on the treatment of knee osteoarthritis with wind-cold-dampness retention type was clear.In the short term(2 weeks after treatment),TCM syndromes: clinical recovery in 4 cases,17 cases were markedly effective,effective in4 cases,5 had no effect,the effective rate was 83.3%;clinical efficacy: clinical control in 3cases,18 cases were markedly effective,effective in 5 cases,4 had no effect,the effective rate was 86.73%.Term(4 weeks after withdrawal)TCM syndromes: clinical recovery in 6cases,14 cases were markedly effective,effective in 7 cases,3 had no effect,the effective rate was 90.0%;clinical efficacy: clinical control in 5 cases,16 cases were markedly effective,effective in 7 cases,2 had no effect,the effective rate was 93.3%,all higher than the control group.The two groups of efficacy evaluation by ranking sum test,P>0.05,the differences were not statistically significant,which suggested that Tong Bi Fumigation Decoction is equivalent to Didofenac Diethylamine Gel.Conclusions:(1)Tong Bi Fumigation Decoction can effectively improve the clinical symptoms of knee osteoarthritis,and its efficacy is equivalent to Didofenac Diethylamine Gel.(2)Tong Bi Fumigation Decoction is effective,simple,economical and practical in treating knee osteoarthritis of wind-cold-dampness retention type.Patients are easily accepted and worthy of clinical promotion.
