Objective:The aim of the present study was to explore the diagnosis and treatment of tension-type headache(TTH)based on ICHD-3 beta in a multicenter based sample from Chongqing,China.Methods: Clinical characteristics and demographic were systematically and prospectively collected from March 2014 to December 2015 in 15 participating hospitals in Chongqing,using a semi-structured face-to-face interview.All patients were asked to complete a headache diary at least 4 weeks.Follow-up was conducted through direct or semi-structured telephone interview after starting treatment.Results:A total of 150 patients(97 female/53 male,44.56±11.90 years old)were diagnosed with TTH based on ICHD-3 standard criteria.Patients with CTTH(n=84)were more likely to be older,female,lower educational level,more analgesics and sleep disturbance,compared to them with ETTH(n=66).Among them,114(76%)patients are diagnosed with TTH using alternative criteria.The distributions of age,gender,marriage,educational level and BMI for patients with TTH were not significantly different between those two diagnosed groups.Only one patients was excluded by the alternative criteria because of unfulfilling three of the four characteristic,and other 35 patients having the associated symptom: nausea(n=6),photophobia(n=1),phonophobia(n=28).All patients with TTH fulfilled mild or moderate intensity.And 98.0% patients fulfilled non-pulsating headache quality,99.3% not aggravated by routine physical activity according to standard criteria.Conclusion:Non-pulsating quality and not aggravated by routine physical activity may be the two important points to screen patients with complaint of headache.The alternative criteria of ICHD-3 for TTH is restrictive in the associated symptoms.Nausea would be removed for its role in the overlapping area of CTTH and CM.
