| Objective:With accelerating pace of life and increasing work pressure, the prevalence rate of tension-type headache (TTH) is gradually increased, which affected the normal production and human life. The etiology and pathogenesis of TTH is not fully understood, so the western medicine is lack of specific drugs. Most of traditional Chinese medicine(TCM) research were concentrated in experience summary and reports. So far, there is no epidemic investigation of TTH with TCM constitution. Tutor can get a good curative effect in clinical treatment in using Qianzheng Powder. So, this study is divided into two part.Experiment one by investigating TCM constitution of TTH patients and correlative factors, to explore the relationship between the occurrence of TTH and TCM constitution and influencing factors, expecting to provide new ideas for prevention and treatment of TTH.Experiment two by observing the clinical efficacy of Qianzheng Powder in treating of wind-phlem type TTH, to evaluate the effectiveness and safty of Qianzheng Powder, expecting to provide evidence for clinical application.MethodsExperiment one collected 100 patients of TTH and 100 healthy volunteers from March 2015 to February 2016 in encephalopathy department of First Affiliated Hospital of Guangzhou University of traditional Chinese medicine, investigating general situation, headache situation, related factors, TCM constitution and anxiety and depression, and the data were analyzed by SPSS 19.0 software.Experiment two collected 60 patients of wind-phlem type TTH from March 2015 to February 2016 in encephalopathy department of First Affiliated Hospital of Guangzhou University of traditional Chinese medicine.60 patients were randomly divided into experimental group and control group, each of 30 cases. The treatment group was given Qianzheng Powder(include Baifuzi, Jiangcan, Quanxie, Dilong, Chuanxiong, Fangfeng, Baijili, Gegen, Baishao , Xixin, Zhigancao), daily dose of water decoction.The control group was given merely Eperisone Hydrochloride(50mg, tid). Other related therapeutic drugs should be stopped.2 groups were treated for 2 weeks. Afer 2 weeks, observed headache index and TCM symptoms, to evaluate the therapeutic effect and safty. The data were analyzed by SPSS 19.0 software.Results:Experiment one showed that single constitution accounted for 52% and complex constitution accounted for 48% headache group.9 kinds of constitution distribution from more to less followed by Yang deficiency, Qi deficiency, phlegm dampness, Qi stagnation, blood stasis, damp heat, mild nature, Yin deficiency, special intrinsic quality. Normal people had 73% single constitution and 27% complex constitution.9 kinds of constitution distribution from more to less followed by mild nature, Qi deficiency, Yang deficiency, damp heat, phlegm dampness, blood stasis, Yin deficiency, Qi stagnation, special intrinsic quality in normal people. The average score of SAS in headache group was 42.54 ± 7.15, with SDS 39.89±7.35, which was significantly higher than in the normal group. In 15 related factors, sleep satisfaction, eating patterns, negative feelings were statistically difference between two groups. By logistic regression analysis, mild nature was protective factor of TTH, well phlegm dampness constitution and Qi stagnation constitution were dangerous factors of TTH. Satisfied to sleep and regular dining were protective factors to phlegm dampness constitution in TTH patients. Negative feelings and regular pressure were dangerous factors of Qi stagnation constitution in TTH patients.Experiment two showed that the total effective rate was 90% in treatment group and 76.7% in control group. The improvement of TCM syndrome was also significant. Suggested that the treatment group had better curative effect both in headacche and TCM syndrome. Both of two groups had no obvious adverse reactions.Conclusion:TTH patients’TCM constitution were more complex,which have less peace constitution and more biased constitution. Mild nature was protective factor of TTH. Phlegm dampness constitution and Qi stagnation constitution were dangerous factors of TTH. Satisfied to sleep and regular dining were protective factors to phlegm dampness constitution in TTH patients. Negative feelings and regular pressure were dangerous factors of Qi stagnation constitution in TTH patients. Anxiety and depression in patients with TTH were higher than those in normal adults, and the scores were different in different constitution patients. Qianzheng Powder was better than control group in reducing headache index and improving TCM syndrome, with no obvious adverse reactions occur. |
